Page 10 of 28 <br />Accessing food assistance such as Electronic Benefit Transfer (EBT) cards and connecting them with <br />food banks or meal services. <br /> Addressing housing needs through 211 and housing navigator connections while keeping cases active <br />and exploring options for sober housing, transitional housing, affordable apartments, or shelters. <br /> Securing healthcare insurance and ensuring that healthcare providers see clients. <br /> Arranging transportation and accompanying clients to appointments as needed. <br /> Connecting with support systems such as mentors, family, friends, peers, recovery groups, job <br />centers, volunteer organizations, and counselors. <br /> Visiting incarcerated clients to maintain engagement while in custody. <br /> Facilitating access to medication-assisted treatment for mental health, substance use disorders, or <br />both. <br /> Attend alongside client court proceedings in effort to provide support during court proceedings. <br /> Participate in competency diversion court or other / alternate court hearings on the behalf of client <br />participation and engagement. <br /> Supporting clients in the criminal justice system, including court appearances, written <br />communication, and follow-ups with prosecutors and defense attorneys. <br /> Holding other agencies accountable by advocating for the clients served. <br />2.5 KEY OBJECTIVES <br />The key objectives of the program are to: <br />1. Actively assist the client with overall recovery from behavioral health conditions. <br />2. Provide proactive care management, case support, peer and mental health services for those <br />referred. This includes active outreach and locating those referred. <br />3. Engage with those incarcerated to create a safe discharge plan in the community. <br />4. Provide the client with a single point of contact for connection to multiple health and social services. <br />5. Maintain active contact with individuals referred. <br />6. Develop relationships with local suppliers and community organizations and function as an integral <br />part of the local community. <br />7. Work to remove barriers that prevent individuals’ access to care, such as transportation needs. <br />8. Care coordination, including scheduling appointments, arranging transportation, conducting <br />appointment reminder calls, and following up to verify service initiation, progress, and need for <br />service adjustment and incorporation into the care plan. <br />9. Track data and outcome measures. <br />10. Actively participate in ongoing evaluation and collaboration with city staff and community partners. <br />11. Actively participate in court proceedings related to those served. <br />12. Create meaningful contacts and connections with individuals served. <br />2.6 SUPPLIER RESPONSIBILITIES <br />The supplier will be expected to provide the following level of service at a minimum: <br />1. Upon employing any new staff, provide the employee’s qualifications and experience for the City of <br />Everett’s review and approval. <br />2. Familiarity with appropriate services for individuals referred by the City of Everett and within its <br />geographic area. <br />3. Provide services to individuals who are referred by the City of Everett Community support team or <br />other city identified personnel. <br />Exhibit C
