The form can be found under "Permitting Resources." Generally, this form shall be submitted prior to Public Works Permit Issuance. Submit the form as a stand-alone document. • The "Small Project Declaration of Covenant for Inspection and Maintenance of Onsite Stormwater BMPs" form may be used for projects with only MR #5 BMPs. Otherwise, projects shall use the "Declaration of Covenant for Inspection and Maintenance of Stormwater Facilities and BMPs" form.
