<br /> <br />enrollment and assessment data, <br />performance outcomes, and financial data <br />must be addressed by the Agency in <br />accordance with the Snohomish County <br />HMIS Policies and Procedures, as <br />described further in Exhibit A. <br />Quarterly Performance Report Review <br />Form <br />Within 14 days after Agency receives <br />each Quarterly Performance Report from <br />the City and Snohomish County. <br /> <br /> <br />V. CORRECTIVE ACTION <br /> <br />A. As part of the terms of this contract, the Agency certifies that the City has the authority to <br />monitor the Project at least annually, and as deemed appropriate, to assess performance, <br />compliance, and project progress. If the Agency has expenditure, or performance issues <br />related to utilization, expenditures, outcomes, or required reports, per agreement HCS-24- <br />66-2440-198 the City will implement the following corrective action: <br />a. The City will provide written notice of issues identified and steps that the City will be <br />taking to address those issues, requirements for agency response, timeframe for <br />taking action, and/or identification of other necessary information to address the <br />issues. Subsequent written document may be provide depending on status of the <br />issues and progress toward resolution. <br />b. A written Corrective Action Plan (CAP) will be developed detailing actions to be <br />completed by the Agency. A written CAP may be a component of the monitoring <br />letter or a separate written document to address any identified issues. The Agency <br />shall provide response to the request for a CAP by the date provided documenting <br />actions in adequate detail for the City to assess whether issues are in the process of <br />being or have been resolved. The City will assess contract and performance issues <br />using all necessary information to determine if the CAP requirements have been met <br />and issues resolved or if further steps are needed. <br />i. Low-Level Intervention which may include more training or technical <br />assistance, monitoring, more frequent reviews of reports or invoices or other <br />activities to address the concerns. <br />ii. Moderate-Level Interventions will address more serious concerns and may <br />include restricting payments, disallowing costs, repayments, or other actions to <br />address concerns. <br />iii. High-Level Interventions will address the most serious concerns and may <br />include contract suspension or termination, legal action, and could result in not <br />being eligible to apply for funding or receive future funding or other actions that <br />address serious concerns. <br /> <br />VI. ADDITIONAL REQUIREMENTS <br /> <br />A. The Agency shall participate in the following activities:
