EVALUATION FOR <br />AGENCY USE ONLY <br />Street (running east -west) and Rockefeller and Oakes (running north -south). <br />Broadways Avenue and Colby Avenue, which both run north -south through Everett <br />are the main arterials serving the Providence Campus and the site. Broadway <br />connects the Everett Central Business District (CBD) to the south to Marysville and <br />Interstate 5 (I-5) to the north. <br />b. Is the site or affected geographic area currently served by public transit? If so, <br />generally describe. If not, what is the approximate distance to the nearest <br />transit stop? <br />The nearest transit stops to the site are provided along Colby Avenue at 13th Street <br />and along Broadway at 12th and 13th Streets. Presently, Everett Transit and <br />Community Transit provide daily bus service along the arterial roadways in the site <br />vicinity. Everett Transit Routes 4, 5, 7, and 29 serve the Providence Campus with <br />stops on Colby Avenue at 13th Street and on Broadway at 12th and 13th Streets. <br />Community Transit Routes 201 and 202 also serve the Campus with stops along <br />Broadway at 12th and 13th Streets. <br />c. How many additional parking spaces would the completed project or non - <br />project proposal have? How many would the project or proposal eliminate? <br />The proposed project would provide 148 parking spaces associated with the medical <br />clinic and 353 parking spaces with the reconfigured parking lot. The reconfigured <br />parking lot would continue to serve the existing Providence Campus uses. No <br />parking would be eliminated as a result of the project. <br />d. Will the proposal require any new or improvements to existing roads, streets, <br />pedestrian, bicycle or state transportation facilities, not including driveways? If <br />so, generally describe (indicate whether public or private). <br />No off -site new or improved transportation facilities would be required as part of the <br />proposed project. The proposal includes on -site improvements to provide pedestrian <br />linkages and off -site connections. <br />e. Will the project or proposal use (or occur in the immediate vicinity of) water, <br />rail, or air transportation? If so, generally describe. <br />The property is not located in the immediate vicinity of water, rail or air <br />transportation. The proposal would not have a use for these alternate forms of <br />transportation. <br />f. How many vehicular trips per day would be generated by the completed project <br />or proposal? If known, indicate when peak volumes would occur and what <br />percentage of the volume would be trucks (such as commercial and <br />nonpassenger vehicles).
