This proposal deals with the removal of petroleum contaminated soils.All health risks and hazards are <br /> included in the project plans and appropriate measures will be taken to meet OSHA and WISHA <br /> requirements to protect work health. Remediation work will be protective of human health and the <br /> environment and potential future risks will be removed by the proposed work. <br /> 1) Describe any known or possible contamination at the site from present or past uses. <br /> The Subject Property is currently contaminated with petroleum products from past use. This proposal <br /> intends for the removal of the material at the Property. <br /> 2) Describe existing hazardous chemicals/conditions that might affect project development and <br /> design.This includes underground hazardous liquid and gas transmission pipelines located <br /> within the project area and in the vicinity. <br /> The proposed project will not be affected by any existing hazardous chemicals/conditions at the Property. <br /> A private locater and ground penetrating radar(GPR)survey was completed to identify all underground <br /> utilities. <br /> 3) Describe any toxic or hazardous chemicals that might be stored, used,or produced during the <br /> project's development or construction,or at any time during the operating life of the project. <br /> Petroleum contaminated soils may temporarily be stored on the project site during the excavation if <br /> the soil is too wet for shipment. The contractor may store small volumes of diesel fuel during site work <br /> for refueling of equipment.All fuels will be stored in a secondary containment area to ensure releases <br /> do not occur. <br /> 4) Describe special emergency services that might be required. <br /> Special emergency services should not be required by the proposal.A health and safety plan will be <br /> completed with the nearest hospital route and appropriate contacts in case of emergency. The health <br /> and safety plan will be completed as part of the requirements under OSHA 40-hour HAZWOP work. <br /> 5) Proposed measures to reduce or control environmental health hazards, if any: <br /> All onsite workers will abide by the project plans, which reduce and control the environmental health <br /> hazards onsite. <br /> b. Noise <br /> 1)What types of noise exist in the area which may affect your project(for example: <br /> traffic, equipment,operation,other)? <br /> Traffic noise is relevant in the near vicinity of the Subject Property(Evergreen Way)but will not affect the <br /> project. <br /> SEPA Environmental checklist(WAG 197-11.960) July 2016 Page 9 of 16 <br />
