PUBLIC WORKS <br /> MEMORANDUM <br /> TO: Teresa Weldon, Planning Department <br /> FROM: Sabrina Fandler, Engineering/Public Services Department <br /> DATE: 4/19/2017 <br /> SUBJECT: Public Works Comments for REVI17-009 <br /> PROPOSAL: Assisted Living, 16-bed `Enhanced services' Facility. The building also includes <br /> 3,000sf of ground floor retail, 14 under-building parking stalls, 20 surface parking <br /> stalls, and related site improvements. <br /> LOCATION: 6502 Evergreen Way <br /> A review of this project by the Engineering/Public Works Department indicates that the following <br /> conditions shall be met: <br /> 1. Construction permits, inspections, and final approvals are required for this project from City of <br /> Everett Permit Services. <br /> 2. A Demolition Permit must be obtained at the Permit Counter prior to any demolition on-site. In <br /> addition, all existing water services will need to be capped at the back side of the existing meter <br /> and a Utility Permit for side sewer cap-off must be obtained with the demo permit to cap the <br /> sewer at the property line prior to demolition of the building. (If not already obtained.) <br /> 3. A Public Works Permit is required for this project. Detailed drawings in accordance with City <br /> Design and Construction Standards shall be submitted to the Public Works Department, <br /> showing site parking layout, landscaping, utilities, storm drainage, temporary construction <br /> erosion control, and all required improvements in the public right-of-way. Public Works <br /> Department approval of these drawings is required prior to any permits being issued. All <br /> improvements shall be completed and approved before the Occupancy Permit is issued/final <br /> approval is granted. <br /> 4. The construction plans must be designed from a site survey performed by a licensed State of <br /> Washington land surveyor. <br /> 5. Temporary erosion control measures for construction activity must be operational prior to <br /> commencement of any clearing or earthwork. <br /> 6. Any grading/fill on this site shall be done so as to not impact the surrounding properties. <br /> 7. Stormwater from this project shall be managed in accordance with current City of Everett <br /> standards. <br /> Page 1 of 3 <br />
