4. City streets and alleys are to be kept clear of dirt and debris at all times during construction. Dust <br /> suppression and street cleaning must occur as directed by the Public Works Inspector. <br /> 5. Lack of comment on the plans submitted with the application does not constitute approval of these plans. <br /> 6. A two year guarantee is required for private retention/detention or other drainage treatment/abatement <br /> facilities serving areas larger than one acre. <br /> 7. A permit is required for the destination of the fill. <br /> 8. The construction plans must be designed from a site survey performed by a licensed State of Washington <br /> land surveyor. <br /> 9. All exposed ground must be reseeded within 30 days of the completion of construction. If reseeding is <br /> not practical due to weather or seasonal problems, the ground must be covered with mulch as directed by <br /> the City Engineer. <br /> 10. Temporary erosion control measures for construction activity must be operational prior to <br /> commencement of any clearing or earthwork in this case for the work already done and put around the <br /> pile of crushed concrete pile to avoid runoff from the pile. <br /> 11. Stormwater runoff from the stockpile must not cause or contribute to a violation of surface <br /> water quality standards (Chapter 173-201A WAC), ground water quality standards(Chapter 173- <br /> 200 WAC), sediment management standards (Chapter 173-204 WAC), and human health-based <br /> criteria in the National Toxics Rule(40 CFR Part 131.36). Of particular concern is the pH impact <br /> of the crushed concrete material. The applicant shall prepare a Surface Water Pollution Prevention <br /> Plan (SWPPP)to ensure and document that water quality standards are not being violated. The <br /> Washington State Department of Ecology's Construction Stormwater Pollution Prevention Plan <br /> requirements for an inactive site may be used as reference, but application for coverage under the <br /> permit is not necessary. The SWPPP should include: <br /> • Monthly sampling the pH of runoff from the crushed concrete stockpile for one year or until <br /> the pH of the runoff is stabilized within the acceptable range of 6.5 to 8.5, whichever is longer <br /> •Sampling or other investigation to demonstrate that no other pollutants are being generated by <br /> other materials within the stockpile which could violate water quality standards. <br /> •Sampling locations and methodology <br /> •Actions to be taken when pH exceeds the allowable range, including: <br /> -- Sampling weekly, or more frequently, until pH measurements are within the acceptable <br /> Range <br /> --Adaptive measures to be taken to prevent high pH runoff from entering surface waters <br /> --Reporting of the exceedance to the City of Everett and when appropriate to the Washington <br /> State Department of Ecology. Telephone or submit an electronic report to the applicable <br /> Ecology Region's Environmental Report Tracking System(ERTS)number(or through <br /> Ecology's Water Quality Permitting Portal [WQWebPortall). <br /> •Record keeping and reporting requirements including records of sampling results and adaptive <br /> measures taken. Records shall be provided to the City of Everett for approval prior to <br /> ceasing sampling. <br /> 12. As an alternative to sampling the applicant may elect to fully cover the stockpile to limit rainwater <br /> contact with the crushed material.
