Exhibit A (Scope of Work) <br />•Monthly data tracking and reporting, covering call types, interventions, referrals,outcomes, and diversion rates. <br />•Facility access for staff to engage with the CAD and phone systems to facilitate call <br />transfers. <br />•Flexibility to accommodate special program needs. <br />•Collaboration with Sno911 and Everett’s Community Support Manager or designee to <br />develop care plans for frequent 911 callers. <br />•Provide documents audit purposes as needed <br />•Maintain records for a minimum of 7 years <br />•Monthly billing and reporting <br />•Adherence and compliance to additional clauses as required by but not limited to: <br />SAMHSA <br />VI.Staff Expertise <br />Staff must possess: <br />•Technical proficiency in human development, crisis intervention, and behavior <br />management. <br />•Behavioral health expertise with an interdisciplinary approach, ensuring clients are <br />connected to the appropriate resources. <br />•De-escalation skills to manage individuals in crisis. <br />•Cultural competence to understand and serve diverse populations. <br />•Decision-making abilities to assess client progress and recommend actions. <br />•Training in behavioral health, disabilities, substance use disorders, and diverse <br />communities. <br />•Knowledge of safety and emergency procedures as established by the Supplier and the <br />City. <br />•Computer proficiency in Microsoft Word and Excel, as well as CAD systems for call <br />tracking. <br />VII.Requirements for reporting and data tracking <br />Client tracking and data reporting must be provided to the City of Everett’s Community Development <br />Department- CARE team. Key data points may include but are not limited to the following. The City <br />reserves the right to request additional information as needed under title 45. <br />•Number of unique individuals served. <br />•Number of referrals from Sno911 <br />•Number of individuals referred to Everett’s Crisis Responders <br />•Number of individuals referred by Everett’s CARE Team <br />VIII.Budget Narrative <br /> (For a full budget schedule of expenditures, refer to Exhibit B of the agreement.) <br />IX. Privacy Protection. To the extent applicable, Service Provider shall appropriately safeguard <br />Protected Health Information ("PHI"), if any, that is created, received, maintained, or transmitted on <br />behalf of the City in compliance with the applicable provisions of HIPAA and the HITECH Act. If <br />determined necessary by either party, the parties will execute Business Associate Agreement.
