A copy of each intersection’s traffic control plan. 4 <br /> 5 <br />3. Haul routes from supplier facilities, and locations of temporary parking and 6 <br />staging areas, including return routes. Describe the complete round trip as it 7 <br />relates to the sequencing of paving operations. 8 <br /> 9 <br />4. Names and locations of HMA supplier facilities to be used. 10 <br /> 11 <br />5. List of all equipment to be used for paving. 12 <br /> 13 <br />6. List of personnel and associated job classification assigned to each piece of 14 <br />paving equipment. 15 <br /> 16 <br />7. Description (geometric or narrative) of the scheduled sequence of planing and of 17 <br />paving and intended area of planing and of paving for each day’s work, must 18 <br />include the directions of proposed planing and of proposed paving, sequence of 19 <br />adjacent lane paving, sequence of skipped lane paving, intersection planing and 20 <br />paving scheduling and sequencing, and proposed notifications and coordinations 21 <br />to be timely made. The plan must show HMA joints relative to the final pavement 22 <br />marking lane lines. 23 <br /> 24 <br />8. Names, job titles, and contact information for field, office, and plant supervisory 25 <br />personnel. 26 <br /> 27 <br />9. A copy of the approved Mix Designs. 28 <br /> 29 <br />10. Tonnage of HMA to be placed each day. 30 <br /> 31 <br />11. Approximate times and days for starting and ending daily operations. 32 <br /> 33 <br />5-04.3(14)B3 Pre-Paving and Pre-Planing Briefing 34 <br />At least 2 Working Days before the first paving operation and the first planing operation, 35 <br />or as scheduled by the Engineer for future paving and planing operations to ensure the 36 <br />Contractor has adequately prepared for notifying and coordinating as required in the 37 <br />Contract, the Contractor must be prepared to discuss that day’s operations as they relate 38 <br />to other entities and to public safety and convenience, including driveway and business 39 <br />access, garbage truck operations, transit operations and working around energized 40 <br />overhead wires, school and nursing home and hospital and other accesses, other 41 <br />Contractors who may be operating in the area, pedestrian and bicycle traffic, and 42 <br />emergency services. The Contractor, and Subcontractors that may be part of that day’s 43 <br />operations, must meet with the Engineer and discuss the proposed operation as it 44 <br />relates to the submitted planing plan and paving plan, approved traffic control plan, and 45 <br />public convenience and safety.
