(ASI) <br />Microbiological Testing, Research and Consulting <br />Page 4 of 4 <br /> <br />PRICE QUOTATION # 2025-0227-03P <br /> <br />Client: Everett, City of <br />Contact: Aaron Chan <br />Project: Cryptosporidium & Giardia Monitoring <br />Date: 27 FEB 2025 <br /> <br />TERMS AND CONDITIONS <br />1) Shipment of Samples to the Laboratory: <br />a. Ship refrigerated samples (reusable ice packs) on the day of collection via overnight, priority delivery. ASI is not responsible for <br />timely delivery of samples shipped using our FedEx account. <br />b. Compliance samples are typically received Monday-Thursday and Friday by arrangement. Receipt of samples on Saturday or <br />holidays require prior approval and arrangement and may incur additional costs. <br />2) Turnaround Time (TAT): Standard turnaround time for the above analyses is 10 business days from receipt of samples. <br />3) Lost or damaged sampling supplies or equipment: Replacement/repair costs will be billed to the Client. In the event of lost or <br />damaged supplies resulting in no billable unit analytical fee, ASI reserves the right to invoice for supplies even if these costs were <br />built into the analytical unit cost and were not specified as line items in the service agreement. <br />4) Return of rental equipment: Equipment not returned to Analytical Services, Inc. within thirty (30) days of the end of sampling will <br />automatically become the property of the Client. Purchase price and all rental charges will be billed to the Client. <br />5) Return of Client Coolers: If cooler return is required, there may be an additional fee. <br />6) Sample Disposal: Hazardous waste disposal, if required, will be assessed an additional fee. <br />7) Written Authorization: A signed copy of these Terms and Conditions is required prior to initiation of any order. <br />8) Payment Terms: <br />a. Billable account status requires approval of ASI’s Accounts Receivable Dept. <br />b. Payment terms for billable projects are net thirty (30) days with 1.5% assessed monthly to overdue accounts. <br />c. Other payment terms require written pre-approval from Analytical Services, Inc.’s Accounts Receivable Dept. <br />9) Restrictions: <br />a. Above pricing is current and good through 31 DEC 2024. <br />b. State law requires us to charge 6% Vermont Sales & Use Tax for Sampling Equipment rented to Vermont residents/businesses. <br />10) Additional Subsamples: Certain matrices of water require one or more additional subsamples to be processed to meet LT2 <br />requirements. This results in additional cost as noted above. Due to analytical procedure hold times, client approval is assumed for <br />up to three additional subsamples if needed – unless written instructions to contrary are provided in advance. <br />11) Additional Analyses: Some water matrices may plug the filter prior to passage of 10L; in this situation a second filter is used per LT2 <br />requirements and client approval is assumed – unless written instructions to contrary are provided in advance. <br />12) Results: Samples will be analyzed as described above. Some samples may contain inhibitory substances which cause matrix <br />interference even when all QA/QC criteria are met; such analyses are billable. <br />13) Costs of Collection: In the event that collection of outstanding invoices becomes necessary, client agrees to pay the reasonable <br />costs thereof including reasonable attorney’s fees and interest. <br />14) Restocking Costs: Costs associated with re-stocking client supplies, re-collection and shipment of samples, etc., which may be <br />required due to circumstances beyond ASI’s control, are the responsibility of the client. Examples include samples arriving out of <br />hold time, out of temperature specifications, etc. For LT2, ASI recommends that extra supplies are provided ahead of time to enable <br />immediate re-sampling, according to the client’s monitoring schedule. Otherwise, supplies for re-sampling would need to be shipped <br />out immediately and the client will be billed for all charges associated with both the original and repeat samples. Costs for actions <br />required by events that may occur while the sample is within ASI’s control, will be born by ASI. An example would be a laboratory <br />accident with a sample under analysis. Supplies for re-sampling would be shipped out immediately and the client will be billed, at <br />the rates specified herein, only for charges associated with the valid sample analysis.
