2025 FEDERAL OVERLAY, W MUKILTEO BLVD – GLENWOOD AVE TO DOGWOOD DR <br /> PAGE SP49 OF SP123 SPECIAL PROVISIONS <br />WORK ORDER #3842 April 15, 2025 FEDERAL AID# NHPP-2776(011) <br />1-07.18(5)K.RTF 1 <br />1-07.18(5)K Professional Liability 2 <br />(December 30, 2022 APWA GSP) 3 <br /> 4 <br />The Contractor and/or its subcontractor(s) and/or its design consultant providing construction 5 <br />management, value engineering, or any other design-related non-construction professional 6 <br />services shall provide evidence of Professional Liability insurance covering professional 7 <br />errors and omissions. 8 <br /> 9 <br />Such policy shall provide the following minimum limits: 10 <br />$1,000,000 per claim and annual aggregate 11 <br /> 12 <br />If the scope of such design-related professional services includes work related to pollution 13 <br />conditions, the Professional Liability insurance shall include coverage for Environmental 14 <br />Professional Liability. 15 <br /> 16 <br />If insurance is on a claims-made form, its retroactive date, and that of all subsequent 17 <br />renewals, shall be no later than the effective date of this Contract. 18 <br /> 19 <br />1-07.23.GR1 20 <br />Public Convenience and Safety 21 <br /> 22 <br />1-07.23(1).GR1 23 <br />Construction Under Traffic 24 <br /> 25 <br />1-07.23(1).INST1.GR1 26 <br />Section 1-07.23(1) is supplemented with the following: 27 <br /> 28 <br />f1-07.23(1).OPT5.docx 29 <br />(November 4, 2024) 30 <br />Lane, ramp, shoulder, and roadway closures are only permitted as follows: 31 <br /> 32 <br />*** 33 <br />W Mukilteo Blvd – Glenwood Ave to Dogwood Dr 34 <br />Single lane alternating flagging operation will be permitted between 7:00am and 35 <br />10:00pm. The Contractor has been granted a one hour early start from 6:00am 36 <br />to 7:00am for traffic control setup only. Casting adjustments shall be permitting 37 <br />from 7:00am and 9:00pm. 38 <br /> *** 39 <br /> 40 <br />If the Engineer determines the permitted closure hours adversely affect traffic, the 41 <br />Engineer may adjust the hours accordingly. The Engineer will notify the Contractor 42 <br />in writing of any change in the closure hours. Exceptions to these restrictions are 43 <br />listed below and when applicable take precedence over closures listed above. The 44 <br />Engineer may also consider on a case-by-case basis additional exceptions following 45 <br />a written request by the Contractor. 46 <br /> 47 <br />Lane, ramp, shoulder, and roadway closures are not allowed on any of the following: 48 <br /> 49 <br />1.
