1 <br />1 <br />• Achieve three objectives - prevent spills, contain spills that occur, and clean up spills <br />• Identify the name, location, owner, and type of facility <br />• Include the date of initial operation and oil spill history <br />• Name the designated person responsible <br />• Show evidence of approval and certification by the person in authority <br />• Contain a facility analysis <br />Will maintenance, fueling, and/or repair of heavy equipment and vehicles occur on -site? <br />❑ YesX No <br />Will wheel wash or tire bath system BMPs be used during construction? <br />X Yes ❑ No <br />• Concrete Washout (BMP C154) <br />• Vehicles, construction equipment, petroleum product storage/dispensing, and spills: <br />.- Vehicles, equipment, and petroleum product storage/dispensing areas shall be <br />inspected regularly to detect any leaks or spills, and to identify maintenance needs to <br />prevent leaks or spills. <br />o- On -site fueling tanks and petroleum product storage containers shall include <br />secondary containment. <br />:- Heavy equipment operating below the ordinary high water mark of any waterway on - <br />site shall use vegetable -based hydraulic fluid. Spill prevention measures, such as <br />drip pans, shall be used when conducting maintenance and repair of vehicles or <br />equipment. <br />In order to perform emergency repairs on -site, temporary plastic shall be placed <br />beneath and, if raining, over the vehicle. <br />r Contaminated surfaces shall be cleaned immediately following any discharge or spill <br />incident. <br />The location of all discharge or spill incidents shall be recorded on the plans <br />provided in Appendix A of the SWPPP or the site map provided in the Consent <br />Decree documents in Appendix C and a follow-up inspection and/or confirmation <br />sampling shall be performed by the CESCL to verify that cleanup has been <br />completed <br />Will pH -modifying sources be present on -site? <br />❑ YesX No If yes, check the source(s). <br />1 <br />1 <br />1 <br />1 <br />13 <br />1 <br />
