The City of Everett <br />The Floyd & Snider Team Everett Landfill/Tire Fire Site <br />5.2.1 INSPECTION CHECKLIST <br />An Inspection Form is attached as Figure 5-1 that will be used to record site conditions. Areas <br />of concern for the direct contact pathway include: <br />• Areas of erosion or holes in the minimum two -foot clean soil cap <br />• Damaged security fencing surrounding undeveloped portions of the site (including <br />locks and fence materials) <br />• Penetrations of developed area covers, such as significant cracks or holes in <br />pavement, exposed soil in non -landscape areas due to differential settlement, and <br />tears in exposed hydraulic barriers <br />• Exposed hydraulic barriers or areas of erosion or holes in landscape area cover <br />• Areas of differential settlement that may compromise integrity of subsurface <br />membranes, drainage or gas management systems <br />The following circumstances related to the groundwater pathway will also be inspected: <br />• Evidence of need for stormwater management system repairs <br />• Effectiveness of stormwater management facilities to prevent water infiltration into <br />landfilled materials (e.g. below pavement cover or below hydraulic barrier in <br />landscaped areas) <br />• Effectiveness of stormwater management facilities to appropriately convey <br />stormwater for off -site discharge <br />• Evidence of need for leachate collection systems repairs <br />5.2.2 INSPECTION PROCEDURES <br />In order to complete the Inspection Checklist described in section 5.2.1, the following inspection <br />procedures will be utilized: <br />• Visual inspection of undeveloped area soil cap, developed area covers, stormwater <br />management facilities, the leachate collection system, and undeveloped area site <br />security measures. <br />• Interview of site personnel, including, but not limited to, employees of on -site <br />facilities, building managers, and residents. <br />• Photographing, sketching or making a video record of the site. <br />5.2.3 REPORTING REQUIREMENTS <br />Quarterly inspection results will be recorded on the attached Inspection Form (Figure 5-1), <br />which will be submitted to the City of Everett's Site Manager within 10 working days of <br />inspection. Immediate notification of the City is required if the inspection has discovered any <br />areas where landfill cover has been compromised, causing exposure of refuse.
