It will 7 <br />also list estimated quantities, units of measurement, the items of work, and the materials 8 <br />to be furnished at the unit bid prices. The bidder shall complete spaces on the proposal 9 <br />form that call for, but are not limited to, unit prices; extensions; summations; the total bid 10 <br />amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment 11 <br />of addenda; the bidder’s name, address, telephone number, and signature; the bidder’s 12 <br />DBE commitment, if applicable; a State of Washington Contractor’s Registration 13 <br />Number; and a Business License Number, if applicable. Bids shall be in legible figures 14 <br />(not words) written in ink or typed and expressed in U.S. dollars. The required 15 <br />certifications are included as part of the Proposal Form. 16 <br /> 17 <br />The Contracting Agency reserves the right to arrange the proposal forms with alternates 18 <br />and additives, if such be to the advantage of the Contracting Agency. The bidder shall 19 <br />bid on all alternates and additives set forth in the Proposal Form unless otherwise 20 <br />specified. 21 <br /> 22 <br />1-02.6.GR1 23 <br />Preparation of Proposal 24 <br /> 25 <br />1-02.6.INST3.GR1 26 <br />Section 1-02.6 is supplemented with the following: 27 <br /> 28 <br />1-02.6.OPT3.GR1 29 <br />(September 3, 2024) 30 <br />The Bidder shall submit the following supplemental documents with the Bid in accordance with 31 <br />Section 1-02.9: 32 <br /> 33 <br />1. Disadvantaged Business Enterprise Utilization Certification (WSDOT Form 272-34 <br />056). 35 <br /> 36 <br />2. DBE Written Confirmation Form (WSDOT Form 422-031) - For each and every DBE 37 <br />firm listed on the Bidder’s completed Disadvantaged Business Enterprise Utilization 38 <br />Certification, the Bidder shall submit written confirmation from that DBE firm that the 39 <br />DBE is in agreement with the DBE participation commitment that the Bidder has 40 <br />made in the Bidder’s completed Disadvantaged Business Enterprise Utilization 41 <br />Certification. 42 <br /> 43 <br />3. Good Faith Effort Documentation - Bidder must submit good faith effort 44 <br />documentation with the Disadvantaged Business Enterprise Utilization Certification 45 <br />only in the event the Bidder’s efforts to solicit sufficient DBE participation have been 46 <br />unsuccessful. 47 <br /> 48 <br />4. DBE Item Breakdown (WSDOT Form 272-054) The Bidder shall submit a DBE Item 49 <br />Breakdown form defining the scope of work to be performed by each DBE listed on 50 <br />the DBE Utilization Certification. 51 <br /> 52
