2025 FEDERAL OVERLAY, BROADWAY – CALIFORNIA ST TO 18TH ST SPECIAL PROVISIONS <br />FEDERAL AID# STBGUL-2715(012); COE PW# 3841 SP-6 May 6, 2025 <br /> <br />1-02.6.OPT7.GR1 1 <br />(September 3, 2024) 2 <br />Bidder Questionnaire 3 <br />The Bidder shall submit with their Bid a completed Bidder Questionnaire form (WSDOT 4 <br />Form #272-022). This shall be filled out for each firm who submitted a bid or quote in 5 <br />attempt to participate in the project whether they were successful or not and include the 6 <br />following information: 7 <br /> 8 <br />1. Firm name; 9 <br /> 10 <br />2. Firm address including ZIP code; 11 <br /> 12 <br />3. Firm’s status as a DBE or non-DBE; 13 <br /> 14 <br />4. Race and gender information for the firm’s majority owner; 15 <br /> 16 <br />5. NAICS code applicable to each scope of work the firm sought to perform in its 17 <br />bid; 18 <br /> 19 <br />6. Age of the firm; and 20 <br /> 21 <br />7. The annual gross receipts of the firm. The Bidder may obtain this information by 22 <br />asking each firm to indicate into what gross receipts bracket they fit (less than 23 <br />$1 million; $1-3 million; $3-6 million; $6-10 million; etc.) rather than requesting 24 <br />an exact figure from the firm. 25 <br /> 26 <br />Failure to return this completed form as part of the Bid Proposal package will cause this 27 <br />Bid to be considered irregular in accordance with Section 1-02.13. A copy of this form is 28 <br />included in the Proposal Forms. 29 <br /> 30 <br />1-02.6.OptionA.RTF 31 <br />1-02.6 Preparation of Proposal 32 <br />(April 22, 2025 APWA GSP, Option A) 33 <br /> 34 <br />The first sentence of the second paragraph is revised to read as follows: 35 <br />All prices shall be in legible figures (not words) written in ink or typed, and expressed in 36 <br />U.S. dollars. 37 <br /> 38 <br />Supplement the second paragraph with the following: 39 <br />4. If a minimum bid amount has been established for any item, the unit or lump sum 40 <br />price must equal or exceed the minimum amount stated. 41 <br /> 42 <br />Delete the last two paragraphs, and replace them with the following: 43 <br />The Bidder shall submit with their Bid a completed Contractor Certification Wage Law 44 <br />Compliance form, provided by the Contracting Agency. Failure to return this certification 45 <br />as part of the Bid Proposal package will make this Bid Nonresponsive and ineligible for 46 <br />Award. A Contractor Certification of Wage Law Compliance form is included in the 47 <br />Proposal Forms. 48 <br />The Bidder shall make no stipulation on the Bid Form, nor qualify the bid in any manner. 49
