& ASSOCIATES, INC . <br /> GEOTECI--INICAL ENGINEERING • ENVIRONMENTAL ENGINEERING <br /> CONSTRUCTION TESTING & INSPECTION <br /> January 2,2024 Project No. 094-23024 <br /> LIMITED HAZMAT SURVEY <br /> LEGION MEMORIAL GOLF COURSE <br /> PRO-SHOP ROOF REPLACEMENT <br /> 144 W MARINE VIEW DRIVE <br /> EVERETT,WASHINGTON <br /> 1.0 INTRODUCTION <br /> This report presents the results of our limited hazardous materials (hazmat) survey for the site located at <br /> the referenced address in Everett, WA. The limited hazmat survey was conducted under the conditions of <br /> Krazan & Associates, Inc.'s (Kaazan's) Proposal No. E23060WAL, dated December 12, 2023. Mr. <br /> Daniel Trefethan of the City of Everett Parks & Facilities department gave written authorization on <br /> December 15, 2023, for Krazan to proceed with the limited hazmat survey. <br /> 2.0 PURPOSE AND SCOPE OF WORK <br /> The purpose of the limited hazmat survey was to identify and quantify the presence of asbestos- <br /> containing material (ACM) and lead-based paint(LBP)within the project scope. The scope of work for <br /> this hazmat survey included conducting a visual survey of the structure and conducting bulk sampling and <br /> analysis of materials suspected to contain asbestos or lead. This survey was performed in accordance <br /> with applicable local, state, and federal regulations. <br /> The Krazan representative collected samples and obtained analytical data for suspect asbestos and/or <br /> lead-containing materials identified on the roof of the strucutre. Once collected, each bulk sample was <br /> sealed in an unadulterated plastic bag to eliminate the possibility of cross-contamination. "Chain-of- <br /> Custody" tracking was followed to maintain sample integrity during handling and data reporting.
