Project No. 094-23024 <br /> Page No.2 <br /> 3.0 BUILDING DESCRIPTION <br /> The scope of work for the hazmat survey was client defined as the roof of the pro shop building only. <br /> The structure is approximately 8,000 square foot and is located at 144 W Marine View Drive, Everett, <br /> WA. The structure is a single-story/vaulted ceiling, masonry-framed structure with concrete slab-on- <br /> grade foundations and masonry walls. <br /> 4.0 INVESTIGATIVE METHODS <br /> 4.1 Survey Protocols <br /> 4.1.1 Asbestos <br /> On December 21, 2023, the Krazan field inspector Mr. Jordan Kain, collected ten (10) samples <br /> representing four (4) types of suspect homogenous materials. Mr. Kain is an Asbestos Building <br /> Inspector, certified under the requirements of the United States Environmental Protection Agency (EPA) <br /> Asbestos Hazard Emergency Response Act(AHERA) regulation 40 CFR 763, Subpart E. A copy of his <br /> certificate is provided in Appendix B. Sample locations for this survey were chosen in a semi-random <br /> fashion with emphasis placed on minimizing damage to the sampled materials. The samples were <br /> collected by carefully removing a small amount of the suspect material in a non-abrasive manner. If <br /> possible, samples were collected from existing damaged areas or loose pieces of materials. Each sample <br /> was placed in a separate sealed plastic bag, and labeled with the project number and sample number. <br /> Refer to the Floor Plan following the text for the bulk sample locations. The laboratory report and chain- <br /> of-custody form for asbestos samples are shown in Appendix A. <br /> 4.1.2 Lead <br /> The Krazan inspector collected two (2) representative samples of paint from the exterior of the building <br /> within the project scope. Once collected, the bulk sample was sealed in an unadulterated plastic bag to <br /> eliminate the possibility of cross-contamination. "Chain-of-Custody"tracking was followed to maintain <br /> sample integrity during handling and transport to the lab. Sampling was representative of all layers of <br /> paint.
