Applicants must notify Ecology if they are aware of contaminated soils and/or <br /> groundwater associated with the construction activity. Provide detailed information <br /> with the NOI (as known and readily available) on the nature and extent of the <br /> contamination(concentrations, locations, and depth), as well as pollution <br /> prevention and/or treatment BM Ps proposed to control the discharge of soil and/or <br /> groundwater contaminants in stormwater. Examples of such detail may include, but <br /> are not limited to: <br /> L List or table of all known contaminants with laboratory test results showing <br /> concentration and depth, <br /> ii. Map with sample locations, <br /> iii. Related portions of the Stormwater Pollution Prevention Plan (SWPPP)that <br /> address the management of contaminated and potentially contaminated <br /> construction stormwater and dewatering water, <br /> iv. Dewatering plan and/or dewatering contingency plan. <br /> 2. Transfer of Coverage Form <br /> The Permittee can transfer current coverage under this permit to one or more new <br /> operators, including operators of sites within a Common Plan of Development, provided: <br /> i. The Permittee submits a complete Transfer of Coverage Form to Ecology, <br /> signed by the current and new discharger and containing a specific date for <br /> transfer of permit responsibility, coverage and liability(including any <br /> Administrative Orders associated with the permit); and <br /> ii. Ecology does not notify the current discharger and new discharger of intent to <br /> revoke coverage under the general permit. If this notice is not given, the <br /> transfer is effective on the date specified in the written agreement. <br /> When a current discharger(Permittee)transfers a portion of a permitted site,the current <br /> discharger must also indicate the remaining permitted acreage after the transfer. <br /> Transfers do not require public notice. <br /> 3. Modification of Coverage Form <br /> Permittees must notify Ecology regarding any changes to the information provided on <br /> the NOI by submitting an Update/Modification of Permit Coverage form in accordance <br /> with General Conditions G6 and G19. Examples of such changes include, but are not <br /> limited to: <br /> i. Changes to the Permittee's mailing address, <br /> ii. Changes to the on-site contact person information, and <br /> iii.
