ATTACHMENT A: STATEMENT OF WORK <br /> <br />Department of Licensing Page 12 of 36 Contract No: K9319 <br /> <br />The Contractor shall provide the services and staff, and otherwise do all things necessary <br />for or incidental to the performance of work, as set forth below: <br /> <br />1. DOL Responsibilities: <br /> <br />A. Provide Examiners with the current hard copy version of the following: <br />• CDL Examiner’s Manual, and any subsequent updates as needed <br />• Skills Test Score Sheets <br />• Test Cue Sheet <br /> <br />B. Provide instructions to Contractor and/or Examiners on any electronic <br />testing equipment and reporting systems, as needed to ensure proper <br />utilization of tools. <br /> <br />C. Adhere to the Centralized Scheduling Procedures: <br /> <br />DOL has three (3) skills test sites: Pasco, Parkland & Everett. <br /> <br />Customers within 50 miles straight-line radius (not driving miles) of a DOL <br />test site are required to test with a DOL examiner. <br /> <br />Customers more than 50 miles from a DOL test site will be released to test <br />with a Third Party Examiner. <br />o The physical address of the training site will be used to calculate 50 miles. <br />o Training site is defined as the address where the equipment is physically <br />located if the school has multiple addresses/offices. <br />If DOL has no examiner availability on preferred test date submitted, after training <br />is complete, the customers can be authorized to schedule with a TPE. <br /> <br />D. Evaluate Examiner’s performance at Contract completion, and at least <br />annually. The evaluation will cover a period ending with an established <br />date. <br /> <br />E. Make the performance report available to the Contractor to include any <br />compliance findings and required corrective action. <br /> <br />2. Contractor Responsibilities: <br /> <br />A. Submit to DOL Contract Manager: <br /> <br />I. The written hiring criteria/selection information process for Examiners, <br />when applicable. <br />II. The Applicant’s name, driver’s license number, driver’s license <br />status and payment information. <br />III. Within 24 hours submit notification of any termination of an Examiner <br />for any reason other than decertification. <br /> <br />B. Enter daily test results via CSTIMS application. Results must be entered <br />within the same day the tests are conducted, in addition to driver’s status <br />and proof of payment/receipt. <br /> <br />C. Allow DOL, FMCSA, or its representatives to conduct random <br />examinations, inspections, and audits with or without prior notice.
