Provide DOL Contract Manager with a list of all additional fees stated in <br />Attachment G: Fee Structure, that may be incurred by the person taking the <br />CDL skills test other than the mandatory CDL skills test fee. <br />I. Prior written approval from the DOL Contract Manager must be <br />received before conducting CDL skills testing. <br />• Additional fees include, but are not limited to: <br />o Equipment rental fees <br />o Bank charge fees <br />o Fuel costs <br />II. Special circumstances: Anything outside the statutory examination <br />fee is considered an additional cost and must have prior approval <br />by DOL in accordance with 308-100-180 WAC. <br />E. Retain a copy of the following at headquarters office per Federal Regulations <br />under C.F.R. <br /> 49 § 383.75(8)(ix): <br />I. A copy of each Third-Party Examiner (TPE) State certificate <br />authorizing the TPE to administer CDL Skills Tests for the classes <br />and types of commercial motor vehicles listed, <br />II. A copy of the current Third-Party agreement, <br />III. A copy of each completed CDL skills test scoring sheet for the <br />previous three calendar years in accordance with the retention <br />schedule. <br />IV. A copy of the TPE’s State approved road test route(s) and, <br />V. A copy of each TPE’s training record. <br /> <br />F. Collect the information from Third Party Examiners or applicants that are <br />the minimum requirements to satisfy DOL requirements. Contractor is <br />prohibited from collecting any additional personal information from Third <br />Party Examiners or Applicants. <br /> <br />3. Examiners must: <br /> <br />A. Prior to certification, pass an FBI National Background for all new <br />examiners, per Federal Regulation C.F.R. 49 § 384.228. A WSP <br />Background check for all examiners is required upon contract renewal. <br /> <br />B. Apply through the Professional Business License System (PBLS) via a <br />system like Secure Access Washington (SAW) account. The instructions <br />are on the DOL webpage. https://dol.wa.gov/driver-licenses-and- <br />permits/commercial-driver-licenses-cdl/apply-be-cdl-third-party-examiner. <br /> <br />C. Successfully complete the DOL’s CDL Third Party Examiner (TPE) core- <br />training program and any supplemental or refresher training course(s) as <br />scheduled or required by DOL. <br /> <br />D. Conform to all requirements stated in the CDL Examiner’s Manual.
