such evidence as the following applicant information filed with the State of Washington Gambling Commission: If the applicant is a for profit owner: 1) Application for License with the State of Washington Gambling Commission including any upgrades of change of license class; 2) Most recent Application for Renewal of License with the State of Washington Gambling Commission; 3) Corporate information, if applicable, including, but not limited to Notification of Stock Ownership Change, Disclosure of Corporate Officers, Stockholders Form; 4) Copy of the current license granted by the State of Washington Gambling Commission; If the applicant is a non-profit owner: 1) Application for License with the State of Washington Gambling Commission including any upgrades of change of license class; 2) Copy of the current license granted by the State of Washington Gambling Commission; (i) That the applicant disclose the name and address of the person, association or organization conducting the bingo games, raffles , social card games and/or amusement games or utilizing, displaying, exhibiting or possessing pull tabs or punch boards with the intent to sell chances thereon, or distributing for sale pull tabs or punch boards as the case may be, and the time (s) and place (s) such activities are intended to be conducted, and such other information which may be required to insure proper identification of such applicant on forms to be provided by the City Clerk. (ii) Upon the fulfillment of the above requirements, the City Clerk shall issue the necessary certification evidencing the registration of such person, association or organization who conducts bingo, raffles, social card games and/or amusement games, or who utilizes, displays, exhibits or possesses pull tabs or punch boards with the intent to sell chances thereon, or who distributes for sale pull tabs or punch boards. The nature of such certification shall be in the form as the City Clerk shall deem appropriate. 3
