Page 16 of 20 <br />FORM 4.03 QUESTIONNAIRE <br />Suppliers must complete this “Questionnaire” providing the information in the same order requested <br />below. Suppliers may emphasize in their narrative any areas of their proposal that they believe exceed <br />our requirements. <br />1. Qualifications and Relevant Experience <br />A. Briefly describe your company. Include how long the company has been in business. <br />B. Describe the qualifications of your company, including specific experience providing services <br />at Boxcar Park and experience providing services at similar venues with challenging <br />environments such as wind, weather, outdoor acoustics, and competing with industrial <br />sounds. <br />C. If awarded this contract, who are you proposing to be the project manager? What is their <br />experience with this work and other aspects pertinent to this project? <br />D. What characteristics most distinguish your organization from your competitors? <br />2. Technical Capability, Approach, and Capacity <br />A. Describe your approach to this project, including the execution, management, and control of <br />the project. <br />B. Provide a conceptual Technical Delivery Plan, including, but not limited to: <br />1. Preliminary equipment list <br />2. Video wall size and specifications, including signal management and reasonable <br />redundancy <br />3. Stage size and site requirements <br />4. Audio system design <br />5. Power plan <br />6. Staffing plan <br />7. Proposed load-in and load-out schedule. <br />C. What is your availability for this project? Please include a statement of other work currently <br />underway or anticipated to be in progress during the project's time frame, and show how you <br />intend to schedule projects so that this project is also accomplished. <br />D. Does your firm intend to subcontract any portion of this contract? If so, please provide the <br />following: name of the firm(s), the percentage of work to be performed by each <br />subcontractor, and a description of the nature of work performed by each. <br />3. Communication and Customer Service <br />A. How do you ensure that all stakeholders are kept informed, and what channels will be used <br />for communication? <br />B. How do you handle changes or adjustments to the project, and what communication <br />protocols will be in place for such changes?
