CHANGE ORDER # <br />Project (Procurement) Number: Small Works Roster Project Title: <br />IFQ #2025-062 Thornton A. Sullivan Park Float Renovation <br />Description of Change: (Attach contractor’s change proposal and cost schedule) (Attach Price or Cost Analysis) <br />BUILDING/LOCATION: 11405 Silver Lake Road, Everett, WA 98208 <br />This change order increases the project price to install a permanent concrete anchor and replace threaded <br />rods on Dock #1. This change order will not extend the contract time. <br />Per Section 2.6 of the SWR, the contractor was required to perform a diving check to ensure that connections <br />to submerged concrete anchors were secure. During the diving check, the contractor was unable to locate the <br />concrete anchor for Dock #1. The anchor is required to hold the dock in place and keep it aligned with the <br />access ramp. Without the anchor, the dock cannot be used and is a safety concern for the public. <br />The dock specifications called for ¾” threaded rods. However, during the dock renovation, the contractor <br />discovered that the holes were for 5/8” threaded rods, and most of the existing rods needed replacement <br />because they were bent or broken. This required the contractor to procure new threaded rods to <br />accommodate this change. The City of Everett and the <br />Contractor further agree that Contract time shall be adjusted to the extent shown above.
