Certificate of Non-Debarment/Suspension (included) <br />4.2 SUGGESTED RESPONSE FORMAT <br />• Standard 8 1/2" x 11" paper <br />• Numbered pages <br />• Typed with a minimum of 12-point font <br />• Responses to the questions asked in form 4.03 <br /> <br />
