These rules include PSCAA Regulation I. Excerpts of <br /> 7 Regulation I are included in the Appendix as it relates to fugitive dust control. The <br /> I 8 Contractor shall submit a dust control plan including dust control measures for any of its <br /> 9 activities related to this contract that may cause dust. This plan shall be submitted to the <br /> 10 engineer prior to commencing any activity at the job site. <br /> I 11 <br /> 12 1-07.6 Permits and Licenses <br /> 13 <br /> I 14 Section 1-07.6 is supplemented with the following: <br /> 15 <br /> 16 (April 2, 2012) <br /> 17 The Contractor shall comply with all United States Coast Guard requirements. <br /> I18 <br /> 19 The Contractor shall contact the Coast Guard at least 30 calendar days in advance of all <br /> 20 work in or near the navigable portion of the waterway and request that a Local Notice to <br /> I 21 Mariners be issued for the waterway at this site. <br /> 22 <br /> 23 The Contractor shall contact the Coast Guard for requirements related to the mooring of <br /> I 24 barges, placement of log booms, and all other equipment that could be a hazard to <br /> 25 waterway users. <br /> 26 <br /> I 27 Provisions shall be made for the removal, on 2 hours notice, of all equipment that would <br /> 28 block or partially block, the navigable portion of the waterway. <br /> 29 <br /> 30 The Coast Guard contact is: <br /> 31 <br /> 32 Bridge Administrator <br /> 33 Thirteenth Coast Guard District <br /> I34 915 Second Avenue Suite 3510 <br /> 35 Seattle, WA 98174-1067 <br /> 36 Telephone: (206) 220-7282 <br /> I 37 <br /> 38 All costs incurred in contacting the Coast Guard and in complying with all the requirements <br /> 39 specified herein shall be included in the contract prices for the items of work involved. <br /> I 40 <br /> 41 All costs in connection with delays in the construction caused by the Contractor's failure to <br /> 42 contact the Coast Guard shall be at the Contractor's expense. <br /> I 43 <br /> 44 Section 1-07.6 is modified to read as follows: <br /> 45 <br /> 46 A City of Everett business license is required prior to commencing construction on this <br /> 47 contract. <br /> 48 The Contractor shall obtain all necessary permits required by law and the City of Everett. <br /> 49 All general building, electrical, plumbing permits will be issued at no cost to the <br /> I50 Contractor. In addition, the Contractor shall obtain all required permits for waste <br /> 51 disposal sites.
