I 1 1-07.5(4)Air Quality I 2 3 Section 1-07.5(4) is modified to read as follows: 4 I 5 The Contractor shall comply with all rules of the Puget Sound Clean Air Agency 6 (PSCAA) (800-552-3565). These rules include PSCAA Regulation I. Excerpts of 7 Regulation I are included in the Appendix as it relates to fugitive dust control. The I 8 Contractor shall submit a dust control plan including dust control measures for any of its 9 activities related to this contract that may cause dust. This plan shall be submitted to the 10 engineer prior to commencing any activity at the job site. I 11 12 1-07.6 Permits and Licenses 13 I 14 Section 1-07.6 is supplemented with the following: 15 16 (April 2, 2012) 17 The Contractor shall comply with all United States Coast Guard requirements. I18 19 The Contractor shall contact the Coast Guard at least 30 calendar days in advance of all 20 work in or near the navigable portion of the waterway and request that a Local Notice to I 21 Mariners be issued for the waterway at this site. 22 23 The Contractor shall contact the Coast Guard for requirements related to the mooring of I 24 barges, placement of log booms, and all other equipment that could be a hazard to 25 waterway users. 26 I 27 Provisions shall be made for the removal, on 2 hours notice, of all equipment that would 28 block or partially block, the navigable portion of the waterway. 29 30 The Coast Guard contact is: 31 32 Bridge Administrator 33 Thirteenth Coast Guard District I34 915 Second Avenue Suite 3510 35 Seattle, WA 98174-1067 36 Telephone: (206) 220-7282 I 37 38 All costs incurred in contacting the Coast Guard and in complying with all the requirements 39 specified herein shall be included in the contract prices for the items of work involved. I 43 44 Section 1-07.6 is modified to read as follows: 45 46 A City of Everett business license is required prior to commencing construction on this 47 contract. 48 The Contractor shall obtain all necessary permits required by law and the City of Everett. 49 All general building, electrical, plumbing permits will be issued at no cost to the I50 Contractor. In addition, the Contractor shall obtain all required permits for waste 51 disposal sites.
