I <br /> 1 I 2 1-07.9 Wages <br /> 3 <br /> 4 1-07.9(5) Required Documents <br /> 5 I 6 Section 1-07.9(5) is modified by deleting the following paragraph: <br /> 7 <br /> 8 Certified payrolls are required to be submitted by the Contractor to the Engineer, for <br /> 9 the Contractor and all subcontractors or lower tier subcontractors, on all Federal aid <br /> 10 projects and, when requested in writing by the Engineer, on projects funded with only <br /> 11 Contracting Agency funds. <br /> 12 <br /> 13 Section 1-07.9(5) is modified by adding the following: <br /> 14 I 15 Certified payrolls are required to be submitted by the Contractor to the Engineer, for <br /> 16 every subcontractor, or lower tier subcontractors, on all Federal-Aid projects and, when <br /> 17 requested in writing by the Engineer, on projects funded with only Contracting Agency <br /> 18 funds. . 11 <br /> 19 <br /> 20 Payroll reports for the Contractor and every subcontractor shall be submitted weekly to <br /> 21 the Construction Division, Public Works Annex, 3101 Cedar Street, Everett, Washington <br /> 22 98201. The payroll reports shall contain the following information: <br /> 23 1. Name and residence address of each worker. <br /> 24 2. Social Security number of each worker. I <br /> 25 3. Classification of work performed by each worker. The classification must be <br /> 26 specific and match the classification categories listed in the Project Manual. <br /> 27 4. Total number of hours employed each day. <br /> 28 5. Total number of hours employed during the payroll period. <br /> 29 6. Straight time and overtime hourly rate of wages paid to each worker. <br /> II <br /> 30 7. Total or gross amount earned by each worker. <br /> 31 8. Deductions for Medical Aid, FICA, Federal withholding tax, and any other <br /> 32 deductions taken. I <br /> 33 9. Net amount paid each worker. <br /> 34 10. Contractor's (or subcontractor's) name and address. <br /> 35 11. Days and dates worked. 111 <br /> 36 12. Date of final day of pay period. <br /> 37 13. Whether fringe benefits were paid to each worker as part of the hourly wage <br /> 38 rate or whether fringe benefits were paid into an approved plan, fund, or <br /> 39 program. <br /> 40 <br /> 41 Payrolls may be submitted on Federal payroll form WH-347 (or equivalent). The reverse <br /> 42 side of the form contains an affidavit which must be filled out and signed. If the <br /> 43 Contractor's payroll reports are computerized, the computerized reports may be <br /> 44 submitted along with a Statement of Compliance affidavit photo copied from the back of <br /> 45 form WH-347 (or equivalent). <br /> 46 <br /> 47 The first payroll submitted for the work for both the Contractor and each subcontractorI <br /> 48 shall be labeled "Initial". The last payroll submitted for the work for both the Contractor <br /> 49 and each subcontractor shall be labeled "Final".
