I 1 I 2 1-07.9 Wages 3 4 1-07.9(5) Required Documents 5 I 6 Section 1-07.9(5) is modified by deleting the following paragraph: 7 8 Certified payrolls are required to be submitted by the Contractor to the Engineer, for 9 the Contractor and all subcontractors or lower tier subcontractors, on all Federal aid 10 projects and, when requested in writing by the Engineer, on projects funded with only 11 Contracting Agency funds. 12 13 Section 1-07.9(5) is modified by adding the following: 14 I 15 Certified payrolls are required to be submitted by the Contractor to the Engineer, for 16 every subcontractor, or lower tier subcontractors, on all Federal-Aid projects and, when 17 requested in writing by the Engineer, on projects funded with only Contracting Agency 18 funds. . 11 19 20 Payroll reports for the Contractor and every subcontractor shall be submitted weekly to 21 the Construction Division, Public Works Annex, 3101 Cedar Street, Everett, Washington 22 98201. The payroll reports shall contain the following information: 23 1. Name and residence address of each worker. 24 2. Social Security number of each worker. I 25 3. Classification of work performed by each worker. The classification must be 26 specific and match the classification categories listed in the Project Manual. 27 4. Total number of hours employed each day. 28 5. Total number of hours employed during the payroll period. 29 6. Straight time and overtime hourly rate of wages paid to each worker. II 30 7. Total or gross amount earned by each worker. 31 8. Deductions for Medical Aid, FICA, Federal withholding tax, and any other 32 deductions taken. I 33 9. Net amount paid each worker. 34 10. Contractor's (or subcontractor's) name and address. 35 11. Days and dates worked. 111 36 12. Date of final day of pay period. 37 13. Whether fringe benefits were paid to each worker as part of the hourly wage 38 rate or whether fringe benefits were paid into an approved plan, fund, or 39 program. 40 41 Payrolls may be submitted on Federal payroll form WH-347 (or equivalent). The reverse 42 side of the form contains an affidavit which must be filled out and signed. If the 43 Contractor's payroll reports are computerized, the computerized reports may be 44 submitted along with a Statement of Compliance affidavit photo copied from the back of 45 form WH-347 (or equivalent). 46 47 The first payroll submitted for the work for both the Contractor and each subcontractorI 48 shall be labeled "Initial".
