<br /> PRICE AND PAYMENT PROCEDURES
<br /> expenses and materials required for installing concrete reinforcement. Measurement
<br /> shall be based on percent completion of the concrete reinforcement work. Payment shall
<br /> be on the basis of the lump sum price stated in the Bid Form.
<br /> 13. Item 13: Cast-in-Place Concrete, Section 03 30 00. Includes but it not limited to: the
<br /> furnishing of all necessary materials, forms, linings, coatings, grouting, equipment, tools,
<br /> labor, testing, and all incidental expenses and materials required to install the concrete.
<br /> Measurement and payment for this item shall be per cubic yard.
<br /> 14. Item 14: Steel Framing, Section 05 50 00. Includes but not limited to the furnishing of all
<br /> necessary materials, equipment, tools, labor, testing and all incidental expenses and
<br /> materials required to install the steel framing. Measurement shall be based on percent
<br /> completion of the steel framing work. Payment of this item shall be on the basis of the111
<br /> lump sum price stated in the Bid Form.
<br /> 15. Item 15: Furnish and Install New 36"Flap Gate, Section 33 30 00.
<br /> Includes but is not limited to: purchase and installation of a new 36-inch flap gate and
<br /> appurtenances includes removal and delivery of existing gate to City. Measurement and
<br /> Payment for this item shall be per each.
<br /> 16. Item 16: Furnish and Install New 72"Manhole, Section 33 30 00.
<br /> Includes but is not limited to: purchase and installation of new 72" manhole and
<br /> appurtenances. Measurement and Payment for this item shall be per each.
<br /> 17. Item 17: Furnish and Install Gravel Backfill for Bedding, Section 31 23 35. 111
<br /> Work under this pay item includes all materials, labor, equipment, and incidentals
<br /> required to provide,condition,place,and compact gravel backfill material for bedding.
<br /> Payment for this item shall be on the basis of cubic yards of imported material placed as
<br /> bedding,at the unit price stated in the Bid Form. Measurement for this pay item shall be
<br /> determined on the basis of legible copies of certified receipts or other documentation of
<br /> volume from the supplier. I
<br /> 18. Item 18: Furnish and Install Structural Fill (Crushed Surfacing Base Course), Section 31
<br /> 23 35.
<br /> Work under this pay item includes all materials, labor, equipment, and incidentals
<br /> required to provide, haul, stockpile, moisture condition, place, and compact Structural
<br /> Fill,that is not included in other bid items.
<br /> Payment for Structural Fill shall be on the basis of cubic yards of imported material
<br /> placed as structural fill, at the unit price stated in the Bid Form. Measurement for this
<br /> pay item shall be determined on the basis of legible copies of certified receipts or other
<br /> documentation of volume from the supplier.
<br /> 19. Item 19: Furnish and Install Asphalt Paving, Section 32 12 16.
<br /> Includes but not limited to: saw cutting existing asphalt, installation and compaction of
<br /> base material as specified, furnishing and applying soil residual herbicide, furnishing and
<br /> applying tack coat, furnishing and compaction of new asphalt, re-striping to match
<br /> existing,tools, labor,materials and appurtenances,in place per the Contract documents.
<br /> The contractor's payment for this item shall be per ton asphalt installed.
<br /> 20. Item 20: Removal and Disposal of Soil/Waste Materials Managed as Contaminated,
<br /> Section 31 23 35.
<br /> Work under this pay item includes all materials, equipment, labor, and incidentals re-
<br /> quired for removal, handling, and disposal of materials requiring disposal classified by
<br /> PSO6 REROUTE 012000-6
