S <br /> CITY OF EVERETT PUBLIC WORKS DOCUMENT 00 22 13 <br /> SPECIAL PROVISIONS SUPPLEMENTAL INSTRUCTIONS <br /> TO BIDDERS <br /> I <br /> DOCUMENT 00 22 13—SUPPLEMENTAL INSTRUCTIONS TO BIDDERS <br /> 1.1 GENERAL <br /> A. It is the intent of City to award a contract to the lowest responsive and responsible Bidder. <br /> Before award, the Bidder must also meet the supplemental bidder responsibility criteria listed <br /> below. <br /> B. Further,the apparent low Bidder and second apparent low bidder must submit the Bidder <br /> Qualification Statement(DOCUMENT 004513) and any other documentation listed below <br /> to the City within one business day after the bid opening, unless the City's project <br /> manager,in writing,allows additional time. <br /> C. The City reserves the right to require such documentation from other bidders also. <br /> 1.2 SUPPLEMENTAL RESPONSIBILITY CRITERIA <br /> A. Experience <br /> 1. Criteria: <br /> a. The Bidder must have successfully completed at least five (5) Similar Projects <br /> within the ten (10) years prior to bid opening. The Bidder may count Similar <br /> Projects successfully completed by its Tieback Subcontractor to fulfill this <br /> requirement. <br /> b. The Bidder's project manager for the Project must have successfully supervised to <br /> completion at least 3 Similar Projects within the 5 years prior to bid opening. The <br /> Bidder may count Similar Projects successfully completed by its Tieback <br /> Subcontractor's project manager for the Project to fulfill this requirement. <br /> c. The Bidder's on-site supervisor for the Project must have successfully supervised <br /> to completion at least 3 Similar Projects within the 5 years prior to bid opening. <br /> The Bidder may count Similar Projects successfully completed by its Tieback <br /> Subcontractor's on-site supervisor for the Project to fulfill this requirement. (If on- <br /> 1111 <br /> site supervisor and project manager are the same person, each Similar Project <br /> successfully supervised to completion by that person may be counted in criteria 1.b <br /> and 1.c.) <br /> d. The drill operator for the Tieback Work shall have been an employee of Bidder or <br /> the Tieback Subcontractor for a minimum of one year as of the date of bid <br /> opening. <br /> 2. Definitions: <br /> a. A"Similar Project"means a project that meets all of the following requirements: <br /> 1) If the project is a Bidder project,then awarded total contract value to Bidder <br /> must have been at least$0.5 million dollars. Alternatively, if the project is a <br /> Tieback Subcontractor project, then amount of the Tieback Subcontractor's <br /> subcontract must have been at least$0.3 million dollars. <br /> 2) Project work by bidder's own forces included permanent ground anchors. If <br /> the project is a Tieback Subcontractor project, then the Tieback <br /> PSO6 REROUTE 002213-1 <br /> WO NO. —UP3583 MAY 2016 <br />
