1 <br /> CITY OF EVERETT PUBLIC WORKS DOCUMENT 00 22 13 <br /> SPECIAL PROVISIONS SUPPLEMENTAL INSTRUCTIONS <br /> TO BIDDERS <br /> I <br /> Subcontractor's work must have included installation of permanent ground <br /> anchors. <br /> b. "Tieback Work"means the work in section 31 51 00. <br /> c. "Tieback Subcontractor" means the subcontractor listed in the Bidder <br /> Qualification Statement that Bidder will use for the Tieback Work. (Note: if the <br /> Bidder will do the Tieback Work with its own forces, Bidder should not list a <br /> Tieback Subcontractor.) <br /> 3. Documentation: <br /> a. The Bidder shall submit the Bidder Qualification Statement as provided in <br /> DOCUMENT 004513. 111 <br /> b. The City may also use independent sources of information that may be available to <br /> demonstrate whether the Bidder is in compliance with these criteria. <br /> 4. Evaluation: I <br /> a. In evaluating whether these criteria are met,the City may check references for the <br /> previous projects and may evaluate the project owner's assessment of the Bidder's <br /> performance, including but not limited to the following areas: <br /> 1) Quality control; <br /> 2) Safety record; <br /> 3) Timeliness of performance; 111 <br /> 4) Use of skilled personnel; <br /> 5) Management of subcontractors; <br /> 6) Availability of and use of appropriate equipment; 111 <br /> 7) Compliance with contract documents; <br /> 8) Management of submittals process,change orders,and close-out. <br /> 1.3 PROCEDURE <br /> A. Requests for Criteria Modification. I <br /> 1. A Bidder may request that City modify the supplemental bidder responsibility criteria <br /> listed above. <br /> a. This request must be in writing to the City project manager and must be received <br /> by the City project manager at least six business days before the bid opening. <br /> b. The City project manager will evaluate the information submitted by the Bidder <br /> and will respond within three business days after receipt of the request. <br /> c. If City evaluation results in a change of the criteria, the City will issue an <br /> addendum to the bid documents identifying the new or revised criteria. <br /> d. Any protest of a City decision regarding a request for criteria modification must be <br /> in strict conformity with Everett Municipal Code Chapter 3.46, Bid Protest <br /> Procedures. <br /> B. Additional Information. 1 <br /> 1.
