3.3 QA/QC Reviews: An internal Consultant quality assurance/quality control review of deliverables will be <br /> conducted,as well as confirmation that comments received have been addressed.A record of comments <br /> received will be maintained.Response to each comment received will be tracked to confirm that they have <br /> been addressed. <br /> Work Element 4— ENVIRONMENTAL AND LAND USE PERMITTING <br /> During phase 1 design of this project,the SEPA Checklist was prepared and submitted,with a Notice of Planned Action <br /> Determination being issued on April 27,2010. Some historic and cultural resources research was also performed which <br /> will be used to support some of the documentation efforts described below. <br /> This work element will consist of the following: <br /> 4.1 Cultural Resources; The presence of the State funding will require a Historic&Cultural Resources Review <br /> Form EZ1 to meet the requirements of Executive Order 05-05. The Consultant will prepare the Form EZ1, <br /> including an area of potential effect(APE)memorandum,and submit to the City for submittal to the <br /> Department of Archaeology and Historic Preservation(DAHP). The Consultant will review the previous <br /> historic and cultural resources research documentation and use this as the basis of supporting documentation. <br /> Assumptions: <br /> • One round of City review comments have been budgeted. <br /> • The City is responsible for all permit fees. <br /> • No critical areas are present. <br /> • A NPDES Notice of Intent will not be required for this project,as less than one acre of land disturbance will <br /> occur. <br /> Deliverables: <br /> • Draft and Final Form EZ1 and APE <br /> Work Element 5—BIDDING ASSISTANCE <br /> This work element will consist of the following: <br /> 5.1 RFfs: Perteet will provide assistance to the City during the bidding period(assume four weeks)to respond to <br /> Contractor inquiries <br /> 5.2 Addendums: Perteet will provide assistance to the City during the bidding period(assume four weeks)and <br /> prepare addenda as required(up to two assumed). <br /> Everett Downtown Streetscape Improvements—Phase 2 Page 5 Scope of Services 6/23/2016 <br />
