I <br /> POLICE HQ—MASONRY CLEANING AND REPAIR APRIL 15,2016 <br /> I <br /> 3. Proof of Insurance: Insurance certificates and endorsements shall be obtained and <br /> maintained in force in accordance with SECTION 007200, Part 15 "Liability and <br /> Insurance", of the Contract Documents. <br /> 4. Power of Attorney: Attorneys-in-fact who sign bid bonds or contract bonds must file <br /> with each bond a certified and effectively dated copy of their Power of Attorney. <br /> 5. Statement of Intent to Pay Prevailing Wage(L&I Form F700-029-000) and Affidavit <br /> of Wages Paid (F700-007-000) from the Contractor, Subcontractor and agents to the <br /> Subcontractor shall be submitted to the Employment Standards Division, State I <br /> Department of Labor and Industries, Olympia, Washington. If the prime contract is over <br /> $1,000,000, then Contractor also needs to submit L&I Form F700-164-000 (Affidavit of <br /> Wages Paid EHB 2805 Addendum). I <br /> 6. Weekly Statement with Respect to Payment of Wages (U.S.Dept of Labor Form <br /> WH347): Contractors, Subcontractors, and agents to Subcontractors using Payroll Form <br /> WH347)may use State of Compliance found on back of form. <br /> 7. Weekly Statement of Compliance (U.S.Dept of Labor Form WH 348). Contractors, <br /> Subcontractors, or agents to Subcontractors not using Payroll Form WH347 shall attach <br /> the Statement of Compliance Form WH 348 to each payroll. <br /> 8. Approval of Subcontractors: Contractors shall request approval of Subcontractors on a <br /> form provided by the City prior to their working on the Site. <br /> 9. Construction Progress Schedule: To be submitted as required by Section 007200, Part <br /> 5 "Progress and Completion", of the Contract Documents by the Contractor within ten <br /> calendar days, or such time as determined by the City, after the date of receipt of Notice <br /> to Proceed. <br /> 10. Traffic Control Plan: Standard Traffic Control Plans are included in the City's Design <br /> and Construction Standards. If, and to the extent, such plans are not sufficient, complete I <br /> or adequate to support the Bidder's planned means and methods of performing the Work, <br /> the Bidder must develop an adequate, complete and sufficient traffic control plan at its <br /> cost,that shall be submitted for the City's approval prior to construction. Deviation from I <br /> the Standard Plans must be submitted by the Contractor and approved by the City prior to <br /> construction. <br /> 11. Disposal Sites: Provide the City with the location of all disposal sites to be used, and <br /> also provide copies of the permits and approvals for such disposal sites. The Contractor <br /> shall provide the City with copies of all permits for disposal and storage of surplus <br /> materials within ten calendar days after award of the Contract. I <br /> 1.23 Postponement of Opening <br /> The City reserves the right to postpone the date and time for receiving or opening of Bids, or <br /> both, at any time prior to the date and time established in the Notice to Bidders. Postponement <br /> notices shall be provided to Bidders in the form of addenda. <br /> 1.24 Business License <br /> A City of Everett business license is required for the Contractor and Subcontractors performing <br /> Work on this Project. <br /> I <br /> INSTRUCTIONS TO BIDDERS 00 2113 - 8 <br />
