CITY OF EVERETT SPECIAL PROVISIONS <br /> 1 <br /> 5-04.5 Payment <br /> Delete all paragraphs in 5-04.5 and substitute the following: <br /> Payment for bid items of Work completed pursuant to the Contract Documents will be <br /> as described in Division B - Bid Item Descriptions of these Special Provisions and <br /> Section 1-09 MEASUREMENT AND PAYMENT of the Standard Specifications. <br /> 5-04.5(1) Quality Assurance Price Adjustments <br /> 5-04.5(1)B Price Adjustments for Quality of HMA Compaction <br /> Delete all paragraphs in 5-04.5(1)B and substitute the following: <br /> The maximum CPF of a compaction lot is 1.00. <br /> For each compaction lot of HMA when the CPF is less than 1.00, a <br /> Nonconforming Compaction Factor (NCCF) will be determined. THE <br /> NCCF equals the algebraic difference of CPF minus 1.00 multiplied by 40 <br /> percent. The Compaction Price Adjustment will be calculated as the <br /> product of the NCCF, the quantity of HMA in the lot in tons and the unit <br /> contract price per ton of the mix. <br /> Supplement Division 5 of the Standard Specifications by adding the following: <br /> ' 5-06 PAVEMENT PATCHING <br /> 5-06.1 Description <br /> This Work shall consist of the reconstruction and patching of trenches and other <br /> excavations in paved streets and other paved areas. <br /> 5-06.2 Materials <br /> ' Provide materials conforming to the requirements specified for the materials in Sections 5- <br /> 04 & 5- 05 of the Standard Specifications except as modified by these Special Provisions. <br /> For HMA pavement patching provide HMA CL 1/2", PG 64-22 as specified in Section 5-04 <br /> ' of the Standard Specifications. <br /> Provide asphalt for temporary pavement patch as either: cold mix asphalt (MC 250) per <br /> Section 9-02 of the Standard Specifications or hot mix asphalt (HMA CL 1/2", PG 64-22). <br /> ' Mineral aggregate of MC 250 shall meet the same requirements as the aggregates used in <br /> HMA CL 1/2", PG 64-22. <br /> For tack coat provide emulsified asphalt grade CSS-1 as specified in 9-02.1(6) of the <br /> Standard Specifications. <br /> For cement concrete pavement patching provide Type Ill (H.E.S.) with minimum 4,000 psi <br /> 28-day compressive strength and having curing time of three days. <br /> Provide crushed surfacing top course used for pavement patching conforming to the <br /> requirements of 9-03.9(3) of the Standard Specifications. <br /> At Contractor's option, provide steel sheets of sufficient strength to safely accommodate <br /> HS-20 loadings in lieu of temporary granular and asphalt materials. <br /> 5-06.3 Construction Requirements <br /> 5-06.3(1) General <br /> ' Schedule pavement patching to accommodate the demands of traffic and perform as <br /> rapidly as possible to provide maximum safety and convenience to public traffic. <br /> Water Main Replacement Q Division 5-SURFACE TREATMENTS July 8, 2016 <br /> -Broadway AND PAVEMENTS <br /> WO No.-UP3612 SP-143 <br />
