administration shall notify the City's police department and the parking enforcement officers of <br /> the school calendar (including holidays, in-service days, and so on) annually. <br /> Section 8: Permit—Application—Issuance. The administration of Everett High School has the <br /> authority to create the form of the application for the Everett High School permit, provided that it <br /> shall include spaces for the following information: applicant's name, address, telephone number, <br /> license plate number, and make, model, and registration of the vehicle for which the permit <br /> during weekdays between the hours of 7 a.m. and 2 p.m. is sought. The application shall also list <br /> the designated parking areas available to permit holders. The application shall also indicate the <br /> nature of the documents used to establish eligibility for the permit, and shall indicate the amount <br /> and date of payment of the permit fee to cover the administrative costs of issuance, if a fee is <br /> required. The applicant(s) shall sign and certify the information furnished on the application. <br /> The application also shall include an agreement, including among other things, the delineation of <br /> the affected areas where permit holders shall not park on Monday through Friday from 7 a.m. to <br /> 2 p.m., except holidays. The agreement printed in the application shall state that by placing the <br /> applicant's signature beneath the agreement portion of the application, the permit holder agrees <br /> to waive the right to park in the parking spaces within the affected areas and consents to <br /> enhanced penalties for violating the conditions of the permit as determined pursuant to Section <br /> 11 of this ordinance. The Everett High School administration shall not issue a permit to an <br /> applicant who has not consented to the conditions of the permit as evidenced by the applicant's <br /> signature beneath the printed agreement. If the permit is issued, the permit is valid for one-year <br /> and is non-transferable. <br /> Where there is more than one family member attending Everett High School authorized to use <br /> the vehicle for which the permit is sought, the application shall include each student or faculty <br /> member's name. All applications must include the signature of a legal or registered owner of the <br /> vehicle to which the permit is to be granted. <br /> Section 9: Permit Fee. The administration at Everett High School is authorized to establish a <br /> permit fee, if any, as a condition of permit issuance. If a fee is collected, the fee shall be the <br /> property of the school district. <br /> Section 10: Permit — Requirements. The City authorizes the administration of Everett High <br /> School to determine the form of the permit to be affixed to the permit holder's vehicle subject to <br /> review by the City's Police Department, provided that the permit shall designate the area for <br /> which it shall be valid, the license number of the vehicle, the serial number of the permit, and its <br /> expiration date. The permit shall include an instruction directing the permit holder to affix the <br /> permit in a permanent fashion to the inner side of and in the lower left-hand corner of the rear <br /> window of the vehicle. The Everett High School administration shall change the color of the <br /> permit annually. <br /> Section 11: Violation—Penalties. <br /> A.
