Fatigue design shall conform to AASHTO Section 11, 16 Table 11-1 using fatigue category Ill. 17 18 Complete calculations for structural design, including anchor bolt 19 details, shall be prepared by a Professional Engineer, licensed 20 under Title 18 RCW, State of Washington, in the branch of Civil 21 or Structural Engineering or by an individual holding valid 22 registration in another state as a civil or structural Engineer. 23 24 All shop drawings and the cover page of all calculation 25 submittals shall carry the Professional Engineer's original 26 signature, date of signature, original seal, registration number, 27 and date of expiration. The cover page shall include the contract 28 number, contract title, and sequential index to calculation page 29 numbers.
