• <br /> ,11#./%cm.Aa <br /> W -FE 7 <br /> ORDINANCE NO. 3295-12 <br /> AN ORDINANCE REGARDING INSURANCE PLANS FOR APPOINTIVE OFFICERS,CLASSIFIED <br /> NON-REPRESENTED EMPLOYEES,AND ELECTED OFFICIALS,AMENDING ORDINANCE No. <br /> 1594-89,AS AMENDED(CHAPTER 2.74 EMC) <br /> WHEREAS, Ordinance 1594-89 includes outdated language related to insurance plans <br /> for the City's appointive officers, classified non-represented employees, and elected <br /> officials; and <br /> WHEREAS, greater administrative flexibility is needed to allow contributions from these <br /> employees and officials toward the cost of their insurance plans; <br /> NOW, THEREFORE, THE CITY OF EVERETT DOES ORDAIN: <br /> Section 1. Ordinance No. 1594-89 § 1.B., as amended by Ordinance No. 2560-01 § 1 <br /> and 2 (Chapter 2.74.020 EMC) which reads as follows: <br /> B. The City shall provide an insurance plan for all Appointive Officers <br /> comprised of the following: <br /> 1. Basic and major medical coverage for the Appointive Officer and his/her <br /> eligible dependents. <br /> 2. A dental plan for the Appointive Officer and his/her eligible dependents. <br /> 3. The face amount of life insurance and the principal sum of accidental <br /> death and dismemberment insurance for an appointive officer shall be <br /> equal to two times the Appointive Officer's basic annual earnings, <br /> rounded to the next higher multiple of$1,000, if not already a multiple of <br /> $1,000 plus $2,000. The maximum face amount and maximum principal <br /> sum shall each be $150,000. <br /> 4. Repealed by Ordinance No. 2560-01. <br /> 5. Long term disability insurance shall be 66 2/3% of the first $7,500 of the <br /> Appointive Officer's pre-disability monthly earnings subject to policy <br /> conditions and limitations. <br /> PAGE 1 OF 5 <br />
