Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Operation School Bell program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> [Complete description of activity to be undertaken including what projects, products or services are to be performed, <br /> where they are to be provided, for whom they are to be provided, how they are to be provided] <br /> All services will be carried out in our facility at 5107 Evergreen Way in Everett by trained Assistance <br /> League of Everett volunteers. Our Operation School Bell Chairs for this year are Rock Dorsey and Diane <br /> Pedack. Ms. Dorsey and Ms. Pedack supervise the activities of the volunteers working directly with the <br /> program. Each year a total of about 100 ALE volunteers participate in Operation School Bell program <br /> delivery. <br /> The in-facility Operation School Bell program serves younger children and is based in our headquarters on <br /> Evergreen Way. Children are transported to our facility by their schools to receive their clothing. From our <br /> supply of new school clothing purchased specifically for the program, children select three outfits of school <br /> clothes, a warm winter jacket, and a week's supply of underwear and socks. In addition they are provided <br /> with a hygiene kit with grooming supplies. For those with greatest need;we provide a$25 voucher for new <br /> shoes for smaller children and a $30 shoe voucher for older boys and girls. Cost for the service averages <br /> about $100 per child depending on the size of the clothing, and this includes the shoe voucher. We anticipate <br /> that we will serve approximately 3,500 children countywide in our Everett facility during the period of <br /> August 1St, 2016 to May 31'2017. In this period, the $11,250 provided by the City of Everett will serve <br /> approximately 112 children,pre-school to eighth grade, living within the City of Everett. <br /> General Administration <br /> [Add description of general administrative services to be performed in support of activities noted above] <br /> Assistance League of Everett has no paid staff. However,the skill,dedication,and professionalism of our <br /> volunteers are the equal of organizations with a full roster of paid employees. As stated above, our Operation <br /> School Bell Chairs for this year are Rock Dorsey and Diane Pedack. Ms. Dorsey and Ms. Pedack are <br /> responsible for the overall direction, scheduling, and supervision of the many ALE volunteers who work in <br /> the program. Because we have no paid staff, our Board President, Joanne Gunderson, fills the role that the <br /> Executive Director would play in other non-profit organizations with paid staff. Our governing board also <br /> includes a Vice President of Philanthropic Programs. This year that position is held by Neena Blackwell, <br /> another long-time ALE volunteer. Ms. Blackwell coordinates the activities of our many philanthropic <br /> programs and ensures that there is no duplication of labor and that agency resources are allocated where they <br /> can be most efficiently and effectively utilized. Our elected, governing Board also includes a Treasurer who <br /> B-1 <br />
