D. Levels of Accomplishment- Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> New school clothing for <br /> low-income Everett 16 units of service per 112 low-income children <br /> children in grades month for seven (7) months preschool to grade 8. <br /> preschool to grade 8. <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 112 low-income children preschool to 112 low-income children preschool to <br /> grade 8. grade 8. <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br /> [Add Provide a list of staff and time commitments to be allocated to the activity specified in I.A. above] <br /> ALE has no paid staff. All of our programs are planned,managed, staffed and evaluated by trained volunteers. <br /> No City of Everett funds will be used for staff salaries or staffing costs of any kind.Every cent of the$11,250 <br /> awarded to the Operation School Bell program will be expended for new school clothing and new shoes for <br /> low-income children living within the City of Everett. <br /> Operation School Bell is our largest program and our chairs,Roxann Dorsey and Diane Pedack will work the <br /> equivalent of a full-time schedule during the period when the program is most active—August 2016 to <br /> March/April 2017. The number of volunteers working on the program is so large that it would impractical to <br /> list all of the hours worked by each volunteer and the tasks performed. Each year more than 100 ALE <br /> volunteers work on some aspect of Operation School Bell,contributing about 20,000 service hours. However, <br /> it is important to reiterate that no CDBG funds will be used for staff costs,overhead or any other expense. All <br /> of the funds will be used to purchase new clothing and shoes for the boys and girls. <br /> II. Time of Performance <br /> B-3 <br />
