• <br /> Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Housing Social Services program in a <br /> manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> The goal of Housing Social Services is to help residents living in senior housing to remain in their <br /> homes (apartments) and participate in the community for as long as possible. The program's primary <br /> objective is to maintain or improve the physical, social, emotional and financial well-being for <br /> residents. <br /> Social Services staff help residents acquire assistance devices, low cost prescriptions, meals-on- <br /> wheels, bathing assistance, legal assistance, transportation assistance, health assistance and <br /> financial assistance. They also provide or arrange presentations on topics such as healthy eating, <br /> wellness, self-care, depression screening, money management and budgeting, and safety. Isolation <br /> is also reduced by providing individual support, enhancing daily living skills and social interaction. <br /> Social Services staff also provide client advocacy services. This assistance can include negotiation <br /> with housing managers when rental or housing compliance issues arise. It includes intervention with <br /> service providers when there is a dispute or error about eligibility for benefits. Advocacy helps <br /> mediate and problem solve when conflict arises between residents. It also ensures that residents <br /> receive the most current information with regard to their individual rights. <br /> Service coordinators maintain a regular schedule with office hours at each apartment building, and <br /> are available by phone.
