A unit of service is a contact with or on behalf of a resident including: outreach, counseling, conflict <br /> resolution, monitoring service delivery, advocacy, information and referral, eligibility screening, and <br /> education for residents, family and staff. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. The City may, at its discretion, extend the term of this Agreement <br /> to allow for the expenditure of unexpended funds. <br /> III.
