Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Meals on Wheels program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Good nutrition and socialization are essential to good health and quality of life for older adults and <br /> people with disabilities. Healthy eating plays a critical role in delaying the onset of chronic disease. <br /> Seniors and people with disabilities who cannot prepare their own meals or have to choose <br /> between food and other basic needs are at greater risk of hospitalization, chronic disease, more <br /> costly long-term care, and displacement from their home. <br /> Senior Services' Meals on Wheels program reduces hunger and food insecurity; promotes <br /> socialization; and promotes the health and well-being of individuals; by assisting them in gaining <br /> access to nutrition and other health promoting services. These services help program participants <br /> delay the onset of adverse health conditions resulting from poor nutritional health. <br /> The reason a person requests Meals on Wheels varies by need. In some cases, Meals on Wheels is a <br /> temporary solution to assist people who are recovering from an acute illness, surgery, or experience a <br /> sudden change in their living situation or support system. Providing meals gives needed nutrition to aid <br /> in their recovery and helps them through a difficult time until they are able to meet their own needs. For <br /> many people, however, Meals on Wheels is the one service they need to be able to remain in their own <br /> homes and they require the service for many years. <br /> Meals on Wheels serves homebound seniors (60+ years old) and people living with a disability (18+ <br /> years old) who are not able to carry out the 'activities of daily living' including, dressing, bathing, <br /> preparing meals, walking, toileting, and hygiene. Meals on Wheels recipients receive weekly deliveries <br /> of up to 7 meals, which they select from a menu of 35 meals. Recipients also receive nutrition <br /> assessments, counseling, and nutrition education tailored to their individual needs. <br /> Once Meals on Wheels program participants begin to receive services they worry less about <br /> having enough food to eat thereby reducing food insecurity. In addition, the program provides <br /> nutrition screening, assessment, education, and counseling, to ensure participants are able to <br /> achieve and maintain optimal nutritional status. Receiving meals help program recipients eat a <br /> more balanced diet by including more fruits and vegetables. After beginning services, meal <br /> recipients state their health has improved and they feel better overall. <br />
