D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Class: 2 hour, adult <br /> education class in the <br /> area of Housing 10 classes per month on 75 classes in Everett <br /> Expertise, Health and average between July 1, 2016 <br /> Wellness, Economic Well and June 30, 2017 <br /> being and Family Life <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 225 Total Snohomish County Residents 125 Everett Residents (adults) <br /> (adults) <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br /> Key College of Hope positions include: <br /> 1. 1.0 FTE Program Manager; former VISTA Member with 15+ years experience in <br /> human services. <br /> 2. 0.1 FTE Director of Social Services; Bachelor's degree in interdisciplinary studies of <br /> social and behavioral health. Over 15 years of leadership experience in serving <br /> families in Snohomish County. <br /> 3. 0.5 FTE Data System Development and Reports; supporting the enrollment, scheduling <br /> and data gathering activities of the program <br /> 4. 0.5 FTE Administrative Assistant; data entry. <br /> A-4 <br />
