• 60% of parent participants self-rate as improved or adequate teen/parent relationship <br /> • 60% of teen participants self-rate as increased or adequate communication skills <br /> • 60%of teen participants self-rate as increased or adequate belief in own ability to make positive <br /> choices <br /> • 60% of teen participants self-rate as increased or adequate teen/parent relationship <br /> General Administration <br /> Cocoon House leadership staff will provide administrative support to ensure the WayOUT program is <br /> operated consistent with contract requirements; maintains fidelity to program models; hire, train, <br /> evaluate, and supervise qualified program staff; provide development and leadership to program <br /> staff; develop and maintain effective working relationships within the community; provide data and <br /> reports as required. <br /> B. National Objectives <br /> The Subgrantee certifies that the activities carried out with funds provided under this Agreement will <br /> meet one or more of the following CDBG National Objectives - 1) benefit low/moderate income <br /> persons, 2) aid in the prevention or elimination of slums or blight or, 3) meet community development <br /> needs having a particular urgency - as defined in 24 CFR Part 570.208. Briefly describe how this <br /> National Objective will be met. <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subgrantee against goals and performance standards <br /> required herein. Substandard performance as determined by the City will constitute non-compliance <br /> with this Agreement. If action to correct such substandard performance is not taken by the <br /> Subgrantee within a reasonable period of time after being notified by the City, contract suspension or <br /> termination procedures will be initiated. <br /> D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Parent/Youth Family 30 seminar hours 364 seminar hours <br /> Seminar <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> A-2 <br />
