Project: California Street Overcrossing Project <br /> Change Order No. 3 <br /> Change Order Effective Date: 1/28/03 <br /> Exhibit A—Description of Changed Work <br /> 1. Remove and reinstall PUD duct bank due to redesign of MSE Wall D footing <br /> elevations, conflicts with tie-backs and changed location of the duct bank. <br /> 2. Extend MSE Wall B from Terminal Ave to Federal Ave. <br /> 3. Per the request of the City of Everett, provide the Park Sign foundation for <br /> Maggie Park. <br /> Added Bid Item— Park Sign Foundation <br /> 4. Provide curb inlets due to redesign of storm sewer on EA Line. <br /> Added Bid Item— Curb Inlets <br /> 5. Per the PUD Limited Use Agreement with the City of Everett, provide <br /> additional drainage, wooden gates and additional scaffolding/fence for safety <br /> issues and access protection to the PUD substation. <br /> 6. Provide 8-inch Ductile Iron Storm Sewer Pipe on Federal Ave. <br /> Added Bid Item— 8-inch DI SD <br /> 7. Extra Work Orders for field redesign of the storm water and water systems at <br /> Terminal Avenue, Everett Avenue and Federal Avenue. <br /> 8. Traffic revisions due to redesign of storm and water systems. <br /> Exhibit B — Justification <br /> 1. The location of the PUD duct bank, as shown on Drawing U-3, had to be <br /> removed to install the footing for MSE Wall D, per Drawing RET-3. The <br /> design location of the penetration of the duct bank through Wall D could not <br /> be installed because of conflicts with the corner tie-backs of the MSE Wall D. <br /> The design elevation of the MSE Wall footing was changed so that duct bank <br /> could be installed without conflict with the MSE Wall tie-backs. The design <br /> footing elevation of Wall D was also changed to avoid conflicts with the <br /> sanitary sewer. Cost $387.60 <br /> 2. After award of contract, it was brought to the city's attention by affected <br /> property owners that an agreed order had been negotiated between the <br /> Department of Ecology and the property owner regarding petroleum <br /> contamination. Retaining wall B was extended beyond its anticipated location <br /> to avoid additional impacts prohibited by this DOE order. This extension <br /> eliminated placement of fill material for the roadway on top of an asphalt cap. <br /> This cap supposedly seals the Exxon/Mobil remediation site, a portion of <br /> which is within the project limits.
