• <br /> 3. The City of Everett Parks Department requested that the contractor to <br /> construct a concrete foundation for the Maggie's Park sign. <br /> Cost $2,527.78 <br /> 4. The designed storm drainage catch basins could not be installed due to <br /> conflicts with the MSE wall's anchorage system on the EA-line on the western <br /> approach to the bridge from STA 14+00 to STA 18+30. Five Type I CBs <br /> were deleted and six curb inlets were added to avoid the conflicts with the <br /> anchorage system. Cost $4,680.00 <br /> 5. Per the COE and PUD Limited Use Permit Agreement, the contractor was <br /> required to install additional drainage and gates between the PUD substation <br /> and COE's Right-of-Way. The Contractor had begun the installation of the <br /> bridge soffit falsework when PUD directed them, on August 28, 2002, to <br /> remove the falsework and to stop work on the PUD easement until the <br /> Limited Use Permit Agreement was signed by the COE. The PUD also <br /> required an additional 7-foot temporary construction fence to be installed on <br /> the soffit work platform until the permanent fence was installed. <br /> Cost $9,742.31 <br /> 6. The City of Everett and the Port of Everett requested the up-sizing of the <br /> storm drainage pipe from 6-inch to 8-inch ductile iron pipe on Federal <br /> Avenue. Cost $6,021.12 <br /> 7. During the installation of the new construction of the water main and storm <br /> sewer revisions, numerous conflicts arose due to unmarked or mismarked <br /> utilities and design elevation /field elevation differences. <br /> a. During installation of the storm sewer from Curb Inlet#12 to Catch Basin <br /> #5, the newly installed, redesigned water main was found to be at the <br /> same elevation. A portion of the water main was removed in order to <br /> install the gravity flow drainage system and then reinstalled to avoid the <br /> conflict with the storm sewer. <br /> b. The contract drawings indicate the raising of the sewer manholes to be <br /> completed by installing a precast riser. The existing manholes were not <br /> precast, but tongue and groove manholes. The City of Everett directed <br /> the Contractor to install the precast risers with a Type 4000W concrete <br /> collar. <br /> c. Redesigned water line to avoid conflict with storm drainage line from <br /> CB#1 to CB#2 and existing 24-inch wood stave concrete encased pipe. <br /> d. Redesigned Fire Hydrant#2 connection due to conflict with existing <br /> Verizon duct bank. <br /> e. Existing storm sewer was located 17 feet from the design location which <br /> changed the location of CB#9 and the newly designed 48" Stormceptor. <br /> f. Abandoned existing, unmarked catch basins on Terminal and Federal <br /> Avenues. <br /> g. Due to design elevations differences, existing manholes had to be raised <br /> which were not included in the contract documents. <br /> Exhibit A <br /> T1 <br />
