Supplemental Agreement No. 6 2707 Colby Avenue, Suite 900 Everett,WA 98201 Agreement Number Project Number Phone (425)252-7700 Project Title New Maximum Amount Payable $1,562,908 Description of Work I-5/41st St. I/C Access Point Decision Report— Phase III The Local Agency of City of Everett desires to supplement the agreement entered into with Perteet Engineering, Inc. and executed on June 14, 1999 as well as Supplemental Agreement Nos. 1,2, 3,4 and 5 and identified as Agreement No. all provisions in the basic agreement remain in effect except as expressly modified by this supplement. The changes to the agreement are described as follows: Section 1, SCOPE OF WORK, is hereby changed to read: Add additional services to prepare the Access Point Decision Report for submittal to the Federal Highway Administration for this interchange. I I Section IV,TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days for completion of the work to read: August 31,2003 III Section V, PAYMENT, shall be amended as follows: The maximum total amount payable by the AGENCY to the Consultant, under this AGREEMENT, shall be increased by the sum of One Hundred Eighty-Seven Thousand Four Hundred Eighty Dollars ($187,480)and shall not exceed the amount shown in the heading of this SUPPLEMENTAL AGREEMENT. As set forth in the attached Exhibit A, and by this reference made a part of this supplement.
