This <br />building has been licensed as an Adult Family Home since 27 MAR 2003, license number 347004, licensed as Silver Lake <br />Family Home Care. The licensee has not changed from Bojan Vranjin during that time. Prior to that time, the licensee <br />was David Bodi. <br />I called the following DSHS numbers to try to get more information: <br />• 253-234-6061, Shannon Quinn did the initial inspection at the change of licensee (did not reach but left <br />message). <br />• 360-651-6850, Smokey Point, Field Manager: Lynn Dasher (did not reach but left message). <br />• 360-725-2575, Lacey, Initial Inspections Manager: Amanda Jones (did not reach but left message). <br />• 360-725-6850, Smokey Point, last inspected by DSHS May 2013, DSHS Hang Lu, Adult Family Home Licensor. I <br />spoke to Ms. Lu and found out the following. <br />o Today they are licensed for six beds in six separate bedrooms. Three of the bedrooms are large enough <br />fer two residents, but they are only !icensed for s;x beds in th� Facilities. They naw have tnree smailer <br />bedrooms and three larger bedrooms. <br />o In 2003, they were licensed for six beds. <br />o According to Ms. Lu, the floor plan has not changed since 2000, and "the garage has always been used <br />as bedrooms".
