5 Project: Change Order no. Change Order Effective Date: 2.Add to the contract the following new bid items with unit prices as shown in Exhibit"B". Item#70—Additions&Modifications to the Contract. Create a bid item entitled,"Additions& Modifications to the contract",to pay for agreed lump sum costs or for work completed on a force account basis regarding issues that arose during construction where the contract plans and specifications were silent on how the work was to be paid for. Item#72—Street Sweeping—Rate Corrected. Create a bid item entitled,"Street Sweeping—Rate Corrected"to be paid for on an agreed hourly rate. Item#73—Electrical Issues. Create a bid item on a force account basis to pay for modifications to the contract required during installation of electrical work by the electrical subcontractor.This work is completed and cost totals are$4,044.29. Item#74—Soil Nail Wall. Create a bid item on a per square foot basis to pay for the installation of the redesigned combination soil nail and structural earth wall as identified in the revised"For Construction" drawing sheets 1 thru 4 of 4 by GeoEngineers, Inc.dated 10-21-03. Item#75—Supervision&Temporary Facility. Create a bid item on a per week basis to pay for the additional supervision and temporary facilities required as a result the impact of the redesign of the structural earth wall. Item#76—Survey and Final Cleanup. Create a bid item on a lump sum basis to pay for the additional construction survey for the new structural soil nail wall and final jobsite area cleanup costs. Item#77—Traffic Control Supervisor. Create a bid item on a per hour basis to pay for those hours during the day when the on site nagger performs functions of a traffic control supervisor. Item#78—Quarry Spalls. Create a bid item on a per ton basis to pay for the placement of quarry spalls on the slope above the top of structural wall as shown on the revised"For Construction"drawing sheets 1 thru 4 of 4 by GeoEngineers, Inc. dated 10-21-03. Item#79—Wilder Assistance to Malcolm. Create a lump sum bid item to pay for the labor and • equipment work that Wilder Construction Company will perform to assist it's subcontractor,Malcolm Drilling,during the installation of the soil nail shotcrete wall construction. Item#80—TESC. Create a force account bid item to pay for the TESC work,(Temporary Erosion and Sedimentation Control),that will be used to control sloughing of the exposed slopes during the excavation stage to construct the soil nail shotcrete wall. Item#81-Electrical Costs Due to Structural Wall Change. Create a lump sum bid item to pay for changes to the size of the luminaries in the vicinity of the structural wall and the location where they are being installed as a result of the structural wall redesign. Item#82—Additional Shotcrete Concrete. Create a lump sum bid item to pay for the additional shotcrete concrete required to construct the soil nail structural wall due to unforeseen conditions. Item#83—6'High, 1"x6"Cedar Fence. Create a bid item on a price per linear foot basis to install 6' high, 1"x6"cedar fence. B. Justification of the items being amended or added by this change order Item#2—Traffic Control Labor. The original estimated hours of traffic control labor were 1,260 hours. There were 90 working days allowed in the contract. This represents 14 hours of labor for traffic control on average per day,(or less than 2 flaggers per day). A project such as this would normally average roughly 4 each full time flaggers per day over the time line in the project. Additional hours of traffic control labor will also be required during the extended timeline needed for construction of the soil nail structural wall. v. t 4 Exhibit A Page 5 of 11
