Project: 5 <br /> Change Order no. <br /> Change Order Effective Date: <br /> Item#6—Remove Catch Basin. Plans called for the adjustment of the existing old style catch basins to <br /> grade. These old style catch basin and frames are a one-piece structure that does not allow adjustment of <br /> the frame and grate without destroying the structure of the catch basin. Additionally these old catch basins <br /> have no"catch"or"depth"to them. The decision was made to remove these old style catch basins and <br /> replace them with new inlets. Secondly,often times the plans called for inverts of new pipes tying into <br /> these existing catch basins with grades that were below the bottom catch elevation of the existing old style <br /> catch basins,thus requiring the removal of the existing CB and installation of a new deeper concrete inlet to <br /> accommodate flow elevations. <br /> Item#9—Remove Existing Concrete Extruded Curb. The existing curb and gutter line was overlaid <br /> with asphalt and thus was hidden from direct viewing.The owner assumed the majority of the curb on the <br /> project was a combined curb and gutter section in-lieu-of existing concrete extruded curb. Thus the <br /> quantities of remove existing concrete extruded curb were exceeded and the quantities of remove concrete <br /> curb and gutter were under run. <br /> Item#10—Remove Curb&Gutter. See item#9 above. <br /> Item#18—Gravel Borrow Including Haul. Quantities of gravel borrow including haul under ran the <br /> estimated amounts by 1,237.43 tons. <br /> Item#23—Ornamental Handrail. The bid proposal allowed for 300 linear feet of ornamental handrail <br /> on the project. The plans showed it to be placed on top of the rockeries constructed between 42"d and 43rd <br /> street.City standard drawing detail 332 called for ornamental handrail to be installed on both sides of the <br /> stairways to those businesses on the west side of Evergreen Way between 42"d and 43r1 street. The total <br /> length of ornamental handrail on top of rockeries between 42n1 and 43`d Street is 290 linear feet. The total <br /> linear feet of handrail on both sides of these stairways are 184 linear feet. <br /> In addition to the above-mentioned quantities,approximately 160 linear feet of ornamental handrail will be <br /> installed on the top of the rockery in front of the First Covenant Church and approximately 165 linear feet <br /> of ornamental handrail will be installed on top of the CMU block wall between Evergreen Way and Rucker <br /> Avenue,north of 44'"Street. These additional areas required either ornamental handrail or fencing to be <br /> installed on them per City of Everett standard drawings for safety reasons. <br /> Item#24—Remove& Replace Existing Cement Concrete Steps. The plans showed conceptually where <br /> stairways were to be installed on the project. No elevation differences,(between the top tread and bottom <br /> tread of these stairway areas),were shown on the plans in order to determine the approximate length of <br /> stairway which may be required to achieve the finished elevation change. The stairways were built to <br /> accommodate the existing conditions in the field. The quantities allowed for in the bid proposal were <br /> exceeded. <br /> Item#25—Shoring or Extra Excavation,CL A. This bid item is being deleted as a result of the redesign <br /> of the soil nail structural earth wall. <br /> Item#26—Crushed Surfacing Top Course. The estimated tons of crushed surfacing top course were <br /> exceeded from what was allowed in the original bid proposal do to modifications required in the curb and <br /> gutter grades,do to additional cement concrete sidewalk and cement concrete driveway areas that were <br /> installed,do to additional areas that were prepared for asphalt concrete pavement class B and do to the <br /> modifications required by the structural wall redesign. <br /> Item#27—Asphalt Treated Base. Additional areas were paved above what was reflected in the original <br /> design plans. <br /> Item#29—Asphalt Concrete Pavement,Class B.
