CITY OF EVERETT � <br /> USE PERMIT#REVI 04-030 <br /> Appiicant: Cheryl Miller <br /> 2916 19'h Street <br /> Everett, WA 98201 <br /> Location: 1505 Broadway <br /> Existing Use: Single Family Residence <br /> Proposed Use: Convert the existing single family residence into a massage therapy ciinic. <br /> Zoning: B-2 Community Business <br /> Parking required: 1 per 200 square feet of clinic space. <br /> A permit for the above-described change of use is hereby granted with the following conditions, <br /> which must be met prior to use of the structure for the aroposed use: <br /> 1. Access - O.K. as shown on the site plan reviewed. <br /> �. When access to or within a structure or an area is unduly difficult becau=e of secured openings <br /> or where immediate access is necessary for life saving or fire-fighting purposes, the Fire <br /> Department may recommend or require a key box to be installed in an r3ccessible location. A <br /> rapid entry system may also be desirable in order to avoid unnecessar�damage while making <br /> entry to any building during an emergency situation. <br /> Please call (425) 257-8123 for further information. <br /> 3. Premise Identification - Premises shall be clearly identified by the correct address at the street <br /> entry. Buildings shall be ciearly identified so as to be visible and le:gible from the access road. <br /> Individual apartments and suites, where applicable, shall be clearly and legibly identified from <br /> the access road. <br /> 4. Fire Hvdrants - Locations as planned are acczptable. <br /> 5. Portable Fire Protection -A portable fire extinguisher with a minimum U.L. rating of 2-A:10-B:C <br /> shall be provided within 75 feet of travel on all floors of each buiiding during construction and <br /> prior to occupancy. <br /> 6. Trash Collection - Dumpsters over 1.5 cubic yards capacity shall be locatecl a minimum of 5 feet <br /> from the buiiding [IFC 304.3.3, 2003 Edition]. <br /> 7. Parking must be per City Zoning Code and City Design and Construction Standards for <br /> Development. <br /> 8. The existing residence consists of an S00 square foot rnain floor with a 288 square foot second <br /> floor. The basement on site is finished, however there. is no internal connection between the <br /> ��3 <br />� <br />
