Copies of these regulations and <br /> required forms are available from the Department of Ecology by calling(425)407-7202, <br /> or tol:-Gee in state 1-800-82G-771 G. <br /> 30. A notice of intent to install tanks must be filed with the Department of Ecology at least 30 <br /> dayc prior to installation. Within 30 days aftzr coming into service, the tanks must be <br /> registered with the Department oC Ecology on a state notification form. <br /> 31. All firnis providing tank installation services must be registered with the Department of <br /> Ecology. The supervisor on site during installation must be licensed by passing an exam <br /> administered by the Intemational Fire Code Institute or by another state with which <br /> Washington has a reciprocity agrecment. Within 30 days after coming into scrvice, an <br /> installation checklist form must be provided to the Department of Ecology by the <br /> company that performed the installation work. <br /> GENERAL INFORMATION <br /> 1.
